手法复位配合空气灌肠治疗小儿肠套叠的疗效分析

摘　　要：目的:通过探讨肠套叠空气灌肠的不同整复方法,以期降低患儿在复位过程中并发肠穿孔的风险。方法:回顾性分析2017年1月—2017年12月南方医科大学附属佛山市妇幼保健院收治的132例原发性肠套叠患者的临床诊治资料。20例手法复位配合空气灌肠组的患儿纳为治疗组,112例仅做空气灌肠的患儿纳为对照组,比较两组患儿空气灌肠复位压力、复位成功率以及复位后短期(48 h)复发率。结果:治疗组患儿的复位压力低于对照组,差异有统计学意义(t=5.02,P<0.05);复位成功率以及短期复发比较,差异无统计学意义(χ2=2.05、2.40,P>0.05)。结论:应用手法复位配合空气灌肠治疗小儿肠套叠较单纯空气灌肠整复可降低复位的平均压力,进而降低空气灌肠过程中并发肠穿孔的风险。手法复位配合空气灌肠对治疗小儿肠套叠有一定价值。Objective:To explore different methods of rectification of intussusception by air enema,with the aim of reducing the risk of complicating intestinal perforation in children during the repositioning process.Methods:The clinical consultation data of 132 patients with primary intussusception admitted to the hospital from January 2017 to December 2017 were retrospectively ana⁃lyzed.20 children in the manipulative repositioning with air enema group were included in the treatment group,and 112 children with air enema only were included in the control group.The air enema repositioning pressure,the success rate of repositioning and the short-term(48 h)recurrence rate after repositioning were compared between the two groups of children.Results:The children in the treatment group had lower resetting pressure than the control group,and the difference was statistically significant(t=5.02,P<0.05).There was no statistically significant difference in the rate of successful resetting and in the comparison of short-term re⁃currence(χ2=2.05,2.40,P>0.05).Conclusion:The application of manual repositioning with air enema in the treatment of pediat⁃ric intussusception can reduce the average pressure of repositioning and thus reduce the risk of complicating intestinal perforation during air enema rectification than air enema alone.Manual repositioning with air enema is valuable in the treatment of pediatric intussusception.
