腰椎退行性疾病患者脊柱融合术后康复护理中联合运动疗法的效果分析

摘　　要：目的:探讨腰椎退行性疾病患者脊柱融合术后康复护理中联合运动疗法的应用效果,促进患者腰椎功能改善。方法:选取2020年1月—2021年9月天津医科大学总医院收治的98例腰椎退行性疾病患者作为研究对象,按随机数表法分为对照组和观察组,每组各49例。两组患者均行脊柱融合术治疗,术后均给予康复护理,对照组采用常规运动训练,观察组在常规运动训练基础上联合运动疗法干预。比较两组患者术后直腿抬高角度及腰背伸高度、疼痛程度[视觉模拟评分法(VAS)评分]、腰椎功能[日本骨科协会评估治疗分数(JOA)评分]。结果:两组患者术后1个月直腿抬高角度、腰背伸高度比较,差异无统计学意义(t=0.235、0.112,P>0.05);观察组术后2个月、3个月直腿抬高角度、腰背伸高度均明显高于对照组,差异有统计学意义(t=8.218、11.908、13.737、11.310,P<0.05)。观察组术后1个月、2个月及3个月VAS评分均低于对照组,差异有统计学意义(t=8.724、9.966、13.825,P<0.05)。观察组术后1个月、2个月及3个月JOA评分均高于对照组,差异有统计学意义(t=6.393、8.283、15.830,P<0.05)。结论:腰椎退行性疾病患者脊柱融合术后康复护理中联合运动疗法干预,能够提高患者腰背伸高度,促进患者腰椎功能改善,减轻患者术后疼痛感。Objective:To explore the effect of combined exercise therapy in the rehabilitation nursing of patients with lumbar Degenerative disease after spinal fusion.Methods:98 patients with degenerative lumbar spine diseases admitted to the hospital from January 2020 to September 2021 were selected as study subjects and divided into control group and observation group according to the random number table method,with 49 cases in each group.Both groups were treated with spinal fusion and given postoperative rehabilitation care.The control group was treated with conventional exercise training,while the observation group was treated with combined exercise therapy intervention on the basis of conventional exercise training.The postoperative straight leg raising angle and low back extension height,pain level(visual analog scoring method[VAS]score),and lumbar spine function(Japanese Orthopaedic Association Assessment Treatment Score[JOA]score)were compared between the two groups.Results:There was no statistically significant difference in straight leg elevation angle and low back extension height between the two groups 1 month after surgery(t=0.235,0.112,P>0.05).The straight leg elevation angle and low back extension height in the observation group were significantly higher than those in the control group at 2 and 3 months after surgery,and the differences were statistically significant(t=8.218,11.908,13.737,11.310,P<0.05).The VAS scores at 1 month,2 months and 3 months after surgery were lower in the observation group than in the control group,with statistically significant differences(t=8.724,9.966,13.825,P<0.05).The JOA scores at 1 month,2 months and 3 months after surgery were higher in the observation group than in the control group,with statistically significant differences(t=6.393,8.283,15.830,P<0.05).Conclusion:Combining exercise therapy interventions in the postoperative rehabilitation care of patients with lumbar degenerative diseases after spinal fusion can increase the height of lumbar back extension,promote the impr
