基于IKAP模式的护理结合居家运动干预对妊娠期糖尿病患者自我管理行为及血糖控制的影响

摘　　要：目的探讨基于信息-知识-信念-行为(information-knowledge-attitude-practice,IKAP)模式的护理结合居家运动干预对妊娠期糖尿病患者自我管理行为及血糖控制的影响。方法选取2020年5月—2022年4月于厦门大学附属第一医院杏林分院就诊的86例妊娠期糖尿病(gestational diabetes mellitus,GDM)患者,按随机数字表法分为两组,各43例。对照组采用常规健康教育联合居家运动干预,观察组采用基于IKAP模式的护理结合居家运动干预,干预8周后比较两组患者护理效果。结果观察组干预后饮食、运动、足部护理及血糖监测管理评分均较对照组高,FPG、2 hPG均较对照组低,差异有统计学意义(P<0.05);观察组不良妊娠结局发生率低于对照组,差异有统计学意义(P<0.05)。结论基于IKAP模式的护理结合居家运动干预有利于改善患者自我管理行为及血糖控制效果,改善妊娠结局。Objective To investigate the effects of information-knowledge-attitude-practice(IKAP)nursing combined with home exercise intervention on self-management behavior and blood glucose control in patients with gestational diabetes mellitus.Methods A total of 86 GDM patients admitted to Xinglin Branch,the First Affiliated Hospital of Xiamen University from May 2020 to April 2022 were selected and divided into two groups according to random number table method,43 cases in each group.The control group received routine health education combined with home exercise intervention,while the observation group received nursing combined with home exercise intervention based on IKAP model,evaluated and compared the effects of the two groups of patients after 8 weeks of intervention.Results After intervention,diet,exercise,foot care and blood glucose monitoring and management scores of observation group were higher than control group,FPG and 2 hPG were lower than control group,the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcome in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Nursing combined with home exercise intervention based on IKAP model can improve patients'self-management behavior and blood glucose control effect,and improve pregnancy outcome.
