中药热奄包联合超声雾化疗法在肝肾阴虚型干眼症患者中的应用研究

摘　　要：目的探讨中药热奄包联合超声雾化疗法在肝肾阴虚型干眼症患者中的应用效果。方法选取2021年4月-2022年4月在广西某三甲医院眼科就诊的80例肝肾阴虚型干眼症患者,按照随机数字表法将其分为对照组40例(80眼)和观察组39例(78眼)。对照组予玻璃酸钠滴眼液滴眼,观察组在对照组的基础上予中药热奄包热敷联合超声雾化疗法。观察2组干预前、干预4周后的泪液分泌实验(SIT)结果、泪膜破裂时间(BUT)、角膜荧光素染色(FL)评分、眼表疾病指数评分(ocularsurface disease index,OSDI)、中医证候积分以及2组的临床干预效果。结果干预4周后,观察组SIT长度长于对照组(t=-8.15,P<0.01),BUT长于对照组(t=-9.87,P<0.01),FL、OSDI、中医证候积分均低于对照组(Z=-2.01,P=0.03;t=9.82,P<0.01;t=14.16,P<0.01),临床疗效好于对照组(Z=-2.58,P=0.01)。结论中药热奄包联合超声雾化疗法能够促进泪液分泌、延长泪膜破裂时间,达到治疗肝肾阴虚型干眼症的效果。Objective To explore the application effect of traditional Chinese medicine hot pack combined with ultrasonic atomization therapy on dry eye patients with liver and kidney yin deficiency.Methods Totally 80 patients,with xerophthalmia of liver and kidney yin deficiency in a grade-A tertiary hospital in Guangxi province between April 2021 and April 2022,were selected.They were divided into a control group of 40 cases(80 eyes)and an observation group of 39 patients(78 eyes)according to a random number table.Both groups were given routine health education and sodium hyaluronate eye drops,while the observation group additionally underwent traditional Chinese medicine(TCM)hot pack combined with ultrasonic atomization therapy for 4 weeks.Before and after the intervention,both groups were evaluated using the Schirmer test(SIT),tear break up time(BUT),flou imaging(FL)score,ocular surface disease index(OSDI)and TCM syndrome score.The clinical treatment effect was also compared between the two groups.Results After the intervention,the SIT and BUT of the observation group were significantly longer than that of the control group(t=-8.15,P<0.01;t=-9.87,P<0.01),while the FL,OSDI and TCM syndrome scores were significantly lower than the latter(Z=-2.01,P=0.03;t=9.82,P<0.01;t=14.16,P<0.01).The efficacy of the former was better than the latter(Z=-2.58,P=0.01).Conclusion Traditional Chinese medicine hot pack combined with ultrasonic atomization therapy can treat dry eye patients with liver and kidney yin deficiency by promoting tear secretion,and prolonging the tear film rupture time.
