社区居家养老服务可及性及其影响因素--基于两省五市调研数据的实证分析

摘　　要：借鉴医疗卫生服务可及性概念和“医疗服务利用行为模型”,构建社区居家养老服务可及性的概念模型及影响因素分析框架,基于两省五市的调研数据,使用结构方程模型分析养老信念、个人资源、家庭资源、社区资源对社区居家养老服务可及性的影响以及服务利用行为的中介作用的结果显示,老年人政策了解度、养老保险待遇、子女关心度、服务供给水平和政策落实度直接显著影响社区居家养老服务可及性。政策了解度、子女关心度、服务供给水平、政策落实度可通过服务利用行为的中介作用间接影响服务可及性。由此,提出加强社区居家养老服务政策宣传、提升养老保险待遇水平并缩小待遇差距、鼓励子女为老年人提供情感支持、全面提升服务供给水平、提高服务政策落实度的政策建议,以促进社区居家养老服务可及性的提升。This paper refers to the concept of accessibility of medical and health services and"Medical Service Utilization Behavior Model"to construct a conceptual model of the accessibility of home-community elderly care services and a framework for analyzing the factors affecting the accessibility of home-community elderly care services.Based on survey data of two provinces of five cities,this paper uses structural equation model to analyze the effects of eldercare belief,personal resources,family resources and community resources on the accessibility of home-community elderly care services and the mediating role of service utilization behavior.The results show that the accessibility of home-community elderly care services is directly and significantly affected by the elderly's policy understanding,pension insurance treatment,children's concern,service supply level and policy implementation.Policy understanding,children's concern,service supply level and policy implementation can indirectly affect the accessibility of home-community elderly care services through the mediating effect of service utilization behavior.Therefore,this paper puts forward some policy suggestions,such as strengthening the publicity of community home care service policy,improving the level of pension insurance treatment and narrowing the treatment gap,encouraging children to provide emotional support for the elderly,comprehensively improving the level of service supply,and improving the implementation of service policy,so as to promote the accessibility of home-community elderly care services.
