摘 要:目的:探究异荭草苷对慢性阻塞性肺疾病(COPD)的作用及机制。方法:36只大鼠随机分为对照组、模型组、异荭草苷低、中、高剂量组和地塞米松组,除对照组外,其余组采用香烟烟雾暴露+脂多糖(LPS)法建立COPD大鼠模型,药物干预14 d后,称重计算肺指数,HE染色检测肺组织病理损伤,Smith评分法进行肺损伤评分,肺功能检测仪检测肺活量(FVC)、一秒用力呼气容积(FEV_(1))、一秒率(FEV_(1)/FVC),血气分析仪检测动脉氧分压(PaO_(2))、血氧饱和度(SaO_(2))、动脉二氧化碳分压(PaCO_(2)),ELISA检测肺组织炎症因子IL-6、TNF-α、IL-1β、IL-8水平,试剂盒检测肺组织氧化应激指标MDA和SOD含量,Western blot检测Toll-样受体4(TLR4)、髓样分化因子(MyD88)、核因子-κB p65(NF-κB p65)及p-NF-κB p65蛋白水平,并计算p-NF-κB p65/NF-κB p65。结果:与对照组比较,模型组肺组织出现炎症浸润,肺泡严重塌陷,肺泡壁增厚,肺间质水肿;与模型组比较,异荭草苷中、高剂量组和地塞米松组大鼠肺组织病理损伤明显改善,肺指数和肺损伤评分显著降低(P<0.05),肺功能指标FVC、FEV_(1)、FEV_(1)/FVC显著升高(P<0.05),血气分析指标PaO_(2)水平和SaO_(2)百分比显著升高(P<0.05),PaCO_(2)水平显著降低(P<0.05),肺组织IL-6、IL-1β、IL-8、TNF-α、MDA水平降低(P<0.05),SOD水平升高(P<0.05),TLR4、MyD88蛋白表达下调(P<0.05),p-NF-κB p65/NF-κB p65降低(P<0.05)。结论:异荭草苷可能通过抑制TLR4/MyD88/NF-κB p65减少炎症因子分泌,降低氧化应激水平,改善COPD大鼠肺功能,缓解肺损伤。Objective:To explore effect and mechanism of isoorientin on chronic obstructive pulmonary disease(COPD).Methods:Thirty-six rats were randomly divided into control group,model group,isoorientin low,middle and high doses groups and dexamethasone group.Except for control group,the other groups were given cigarette smoke exposure+lipopolysaccharide(LPS)to establish rat COPD model.After 14 days of drug intervention,lung index was weighed and calculated,lung tissue pathological injury was detected by HE staining and Smith scoring method was used for lung injury score.Lung function tester was used to detect forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1))and FEV_(1)/FVC.Blood gas analyzer was applied to detect arterial partial pressure of oxygen(PaO_(2)),blood oxygen saturation(SaO_(2))and arterial partial pressure of carbon dioxide(PaCO_(2)).ELISA was used to measure levels of inflammatory factors IL-6,TNF-α,IL-1βand IL-8 in lung tissues.Kit was used for detection of levels of oxidative stress indicators MDA and SOD in lung tissues.Western blot was adopted to test protein levels of Toll-like receptor 4(TLR4),myeloid differentiation factor(MyD88),nuclear factor-κB p65(NF-κB p65)and p-NF-κB p65,and p-NF-κB p65/NF-κB p65 was calculated.Results:Compared with control group,model group showed inflammatory infiltration,severe alveolar collapse,alveolar wall thickening and lung interstitial edema in lung tissues.Compared with model group,lung tissue pathological injury in isoorientin middle and high doses groups and dexamethasone group was significantly improved,lung index and lung injury score were significantly decreased(P<0.05),while lung function indicators of FVC,FEV_(1)and FEV_(1)/FVC were increased significantly(P<0.05),and blood gas analysis indicators of PaO_(2)and SaO_(2)percentages were significantly increased(P<0.05),while PaCO_(2)level was significantly reduced(P<0.05),and IL-6,IL-1β,IL-8,TNF-αand MDA levels of lung tissue were decreased(P<0.05),while SOD level was increased(P<0.
