摘　　要：目的:探讨类风湿关节炎(RA)患者外周血CD45^(−)CD31^(−)PDPN^(+)细胞群与红细胞沉降率(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、抗环瓜氨酸肽抗体(抗CCP抗体)、IL-1β、IL-6、TNF-α和IFN-γ的关系。方法:选取福建医科大学附属第一医院56例RA患者和63例健康体检者。根据实验室正常参考范围将ESR、CRP、RF、抗CCP抗体与4种细胞因子的检测结果分为阴性组与阳性组,比较外周血中CD45^(−)CD31^(−)PDPN^(+)细胞群的比例与4种细胞因子、ESR、CRP、RF和抗CCP抗体的关系。采用Mann-Whitney U检验、t检验、χ^(2)检验对指标进行组间比较,CD45^(−)CD31^(−)PDPN^(+)细胞群的比例与年龄的相关性采用Pearson相关性分析。结果:RA患者的CD45^(−)CD31^(−)PDPN^(+)细胞群的比例低于健康体检者(P<0.05);抗CCP抗体阳性组的细胞群比例高于阴性组(P=0.001);CRP阳性组的细胞群比例低于阴性组(P<0.05);TNF-α阳性组的细胞群比例低于阴性组(P<0.05)。结论:RA患者外周血CD45^(−)CD31^(−)PDPN^(+)细胞群比例与抗CCP抗体、CRP和TNF-α有关。Objective:To investigate the relationships between CD45^(−)CD31^(−)PDPN^(+)cells and erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factors(RF),autoantibodies against citrullinated antigens(anti-CCP),IL-1β,IL-6,TNF-α,IFN-γin peripheral blood of patients with rheumatoid arthritis(RA).Methods:A total of 56 patients with RA and 63 healthy controls were included from the First Affiliated Hospital of Fujian Medical University.According to the normal laboratory reference range,the test results of ESR,CRP,RF,anti-CCP antibody and 4 kinds of cytokines were divided into negative group and positive group,respectively.The relationships between the proportion of CD45^(−)CD31^(−)PDPN^(+)cell population in peripheral blood and 4 kinds of cytokines,ESR,CRP,RF and anti-CCP antibody were compared.Indexes were compared between groups by Mann-Whitney U test,t test orχ^(2)test,and the correlation between CD45^(−)CD31^(−)PDPN^(+)cells ratio and age were analyzed by Pearson correlation analysis.Results:The percentage of CD45^(−)CD31^(−)PDPN^(+)cells in RA patients was lower than that in healthy adults(P<0.05);the ratio of the cells in anti-CCP positive group was higher than that in negative group(P=0.001);CD45^(−)CD31^(−)PDPN^(+)cells ratio in CRP positive group was lower than negative(P<0.05),the same as TNF-α.Conclusion:CD45^(−)CD31^(−)PDPN^(+)cells ratio is related to anti-CCP,CRP and TNF-αin peripheral blood of patients with RA.
