摘 要:目的分析并比较六种常见aGVHD生物标志物水平在重型β地中海贫血患儿半相合HSCT前后的差异,明确发生aGVHD之前检测各生物标志物的重要性。方法选取入住我院儿科行半相合HSCT的31例重型β地中海贫血患儿,采用酶联免疫吸附测试(ELISA)检测患儿移植术前、术后及出现aGVHD并治疗后的外周血aGVHD生物标志物(TNFR1、IL-6、IL-8、ST2、Elafin及Reg3α共6种细胞因子)水平。结果本研究队列中的aGVHD发生率为35.48%;6种细胞因子水平在移植前、后均不具有显著性差异(P>0.05);IL-8、TNFR1、ST2、Elafin的水平变化较大,在未发生aGVHD组和发生aGVHD组之间具有显著性差异(P<0.05);患儿在确诊aGVHD后,因子水平变化与aGVHD药物治疗效果相关。结论接受半相合HSCT的重型β地中海贫血患儿的aGVHD生物标志物与aGVHD的发生及治疗效果具有相关性。Objective To analyze and compare the levels of 6 common acute graft versus host disease(aGVHD)biomarkers before and after haploidentical hematopoietic stem cell transplantation(haplo-HSCT)in children withβ-thalassemia major(TM),and to clarify the importance of detecting each biomarker before the occurrence of aGVHD.Methods Thirty-one TM children admitted to the Department of Pediatrics in our hospital and underwent haploidentical HSCT were selected.Enzyme-linked immunosorbent assay(ELISA)was used to evaluate the levels of aGVHD biomarkers(a total of six types of cytokines including TNFR1,IL-6,IL-8,ST2,Elafin and Reg3α)in peripheral blood before transplantation,after transplantation,and post-treatment after developing aGVHD.Results The incidence of aGVHD in this cohort was 35.48%.There were no significant differences in the levels of six cytokines before and after transplantation(P>0.05).The levels of IL-8,TNFR1,ST2 and Elafin varied greatly,and there were significant differences between the no-aGVHD group and the aGVHD group(P<0.05).In addition,after the diagnosis of aGVHD in children,the changes of cytokine levels were related to the effect of drug treatment for aGVHD.Conclusions After haploidentical hematopoietic stem cell transplantation,the biomarkers of aGVHD in children withβ-thalassemia major are correlated with the occurrence of aGVHD and the treatment effect.
