肾活检组织病理诊断中采用石蜡切片免疫荧光染色法的临床价值分析

摘　　要：目的:分析肾活检组织病理诊断中应用新式石蜡切片免疫荧光染色法的可行性与效能。方法:研究开展的时间为2021年1月至2022年12月,共纳入83份肾组织样本,其中10份来源于贵州电力医院,73份来源于贵州医科大学附属医院。对全部样本均实施常规冰冻切片免疫荧光染色诊断与新式石蜡切片免疫荧光染色诊断,将前者列为冰冻组,将后者列为石蜡组,对照两种诊断方式对于常见肾病的诊断效能。结果:肾病理组织活检的阳性检出结果显示,石蜡组23例膜性肾病患者、30例狼疮性肾病患者和30例IgA肾病患者免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)、补体C3、补体C1q的阳性检出率均高于冰冻组,但两组仅在膜性肾病患者IgA、狼疮性肾病患者IgA、IgG、补体C1q的检出率方面存在统计学差异(P<0.05),其他组间对比均无统计学差异(P>0.05)。结论:在肾病患者肾组织病理活检诊断中,新式石蜡切片免疫荧光染色法的诊断效能优于传统冰冻切片免疫荧光染色法,有助于更为准确地检出不同类型的肾病,具有较高的临床应用价值。Objective:To analyze the feasibility and efficacy of the new paraffin section immunofluorescence staining in the pathological diagnosis of renal biopsy.Methods:The study was conducted from January 2021 to December 2022.A total of 83 kidney tissue samples were included,including 10 from Guizhou Electric Power Hospital and 73 from the Affiliated Hospital of Guizhou Medical University.All samples were diagnosed by immunofluorescence staining of conventional frozen sections and new paraffin sections.The former was classified as the frozen group and the latter as the paraffin group,and the diagnostic efficacy of the two diagnostic methods for common nephropathy was compared.Results:The positive detection results of renal pathological tissue biopsy showed that the positive detection rates of immunoglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin G(IgG),complement C3 and complement C1q of 23 patients with membranous nephropathy,30 patients with lupus nephropathy and 30 patients with IgA nephropathy were higher in paraffin group than in frozen group.However,there were statistical differences between the two groups only in the detection rates of IgA,IgG and complement C1q in patients with membranous nephropathy and lupus nephropathy(P<0.05),while there were no statistical differences between the other groups(P>0.05).Conclusion:In the diagnosis of pathological biopsy of kidney tissue in patients with nephropathy,the new paraffin section immunofluorescence staining method has better diagnostic efficacy than the traditional frozen section immunofluorescence staining method,which helps to more accurately detect different types of nephropathy,and has higher clinical application value.
