摘 要:目的分析老年缺血性脑卒中合并原发性失眠患者应用生物反馈放松训练结合耳针对睡眠质量及血清神经递质水平的影响。方法选取2020年9月至2021年9月期间就诊于信阳市中医院的97例老年缺血性脑卒中合并原发性失眠患者作为研究对象,以抽签的方式分组。对照组48例给予生物反馈放松训练,研究组49例增加耳针干预。对比两组患者睡眠结构参数、睡眠质量、血清神经递质水平变化。结果干预后,研究组总睡眠时间(TST)(7.19±1.52)h高于对照组(5.16±1.08)(P<0.001);睡眠潜伏期(SL)(24.37±6.12)min、觉醒时间(AWT)(30.34±6.73)min均低于对照组(40.25±8.23)min、(39.17±8.62)min(P<0.001)。干预后,研究组失眠严重程度指数量表(ISI)(11.86±2.16)分、匹兹堡睡眠质量指数量表(PSQI)(7.92±1.53)分均低于对照组(16.18±3.22)分、(10.52±2.10)分(P<0.001)。干预后,研究组去甲肾上腺素(NE)水平(3.67±0.51)μmol/L低于对照组(3.98±0.58)μmol/L(P<0.05);5-羟色胺(5-HT)水平(90.14±13.29)μg/mL高于对照组(83.16±11.81)μg/mL(P<0.05)。结论生物反馈放松训练结合耳针能够有效减轻患者失眠程度,提高睡眠质量,改善血清神经递质水平。Objective To analyze the effect of biofeedback relaxation training combined with auricular acupuncture on sleep quality and serum neurotransmitter levels in elderly patients with ischemic stroke combined with primary insomnia.Methods A total of 97 elderly patients with ischemic stroke combined with primary insomnia,who were treated in the TCM Hospital of Xinyang from September 2020 to September 2021 were selected.They were divided into control group(n=48)and research group(n=49)by lottery.Biofeedback relaxation training was given to all patients,and auricular acupuncture was additionally given to those of research group.The sleep structure parameters,sleep quality and serum neurotransmitter levels were compared between the two groups.Results After the intervention,the total sleep time(TST)was significantly longer in the research group than that in the control group(7.19±1.52)h VS(5.16±1.08)h,(P<0.001),while the sleep latency(SL)and awakening time(AWT)were significantly lower(SL:[24.37±6.12]min VS[40.2±8.23]min,AWT:[30.34±6.73]min VS[39.17±8.62]min,P<0.001).After the intervention,the scores of insomnia severity index(ISI)and Pittsburgh sleep quality index(PSQI)in the research group were lower than those in the control group(ISI:[11.86±2.16]points VS[16.18±3.22]points,PSQI:[7.92±1.53]points VS[10.52±2.10]points,P<0.001).The norepinephrine(NE)level in the research group was significantly lower than that in the control group(3.67±0.51)μmol VS(3.98±0.58)μmol,(P<0.05),while the 5-hydroxytryptamine(5-HT)level was significantly higher(90.14±13.29)μg/mL VS(83.16±11.81)μg/mL,(P<0.05).Conclusion Biofeedback relaxation training combined with auricular acupuncture can effectively reduce the degree of insomnia,improve sleep quality,and enhance serum neurotransmitter levels,which deserves to be clinically applied.
