TIMP-1、TGF-β1、HIF-1α在膝关节骨性关节炎中的表达及相关性分析

摘　　要：目的探究基质金属蛋白酶抑制剂-1(TIMP-1)、转化生长因子-β(TGF-β1)、中文名低氧诱导因子-1α(HIF-1α)在膝关节骨性关节炎中的表达及相关性。方法选取2021年1月至2022年12月南阳医学高等专科学校第一附属医院收治的膝关节骨性关节炎患者160例,记为病例组,另随机选取于南阳医学高等专科学校第一附属医院进行体检的健康志愿者165例记为对照组。采用酶联免疫法检测TIMP-1、TGF-β1、HIF-1α,分析TIMP-1、TGF-β1、HIF-1α在膝关节骨性关节炎中的表达及意义。结果病例组TIMP-1、HIF-1α、TGF-β1表达分别为(95.36±19.67)μg/L、(3.97±0.03)pg/mL、(76.81±9.27)pg/mL,对照组TIMP-1、HIF-1α、TGF-β1表达分别为(80.15±15.31)μg/L、(4.98±0.16)pg/mL、(42.34±5.03)pg/mL;病例组HIF-1α、TGF-β1表达高于对照组,TGF-β1表达显著低于对照组(P<0.05);与轻度患者相比,中度患者TGF-β1、HIF-1α表达较高,TIMP-1表达较低(P<0.05);与中度患者相比,重度患者TGF-β1、HIF-1α表达较高,TIMP-1表达较低(P<0.05);随着急膝关节骨性关节炎的加重,TGF-β1、HIF-1α表达升高,TIMP-1表达下降(P<0.05);三者联合对膝关节骨性关节炎的诊断价值高于TIMP-1、TGF-β1、HIF-1α单项诊断(P<0.05);以正确诊断指数最大对应的TIMP-1、TGF-β1、HIF-1α表达量作为最佳诊断分界点,分界点TIMP-1表达量为689.35,TGF-β1表达量为5.67,HIF-1α表达量为0.42。TIMP-1、TGF-β1呈现出负相关存在(r=-4.794,P=0.030);TIMP-1、HIF-1α呈现出正相关存在(r=5.169,P=0.024);TGF-β1、HIF-1α呈现出负相关存在(r=-4.810,P=0.029)。结论对膝关节骨性关节炎患者检查TIMP-1、TGF-β1、HIF-1α中发现,TIMP-1、HIF-1α呈现出高表达,TGF-β1呈现出低表达,且临床上可根据上述指标变化进行早期评估膝关节骨性关节炎。Objective To measure the expression levels of tissue inhibitor of metalloproteinase-1(TIMP-1),transforming growth factor-beta 1(TGF-β1)and hypoxia-inducible factor-1 alpha(HIF-1α)in knee osteoarthritis patients and their correlation.Methods A total of 160 knee osteoarthritis patients admitted to the First Affiliated Hospital of Nanyang Medical College from January 2021 to December 2022 were included in case group,and 165 healthy volunteers receiving physical examination in our hospital were randomly selected as control group.The expression levels of TIMP-1,TGF-β1 and HIF-1αwere analyzed by enzyme-linked immunoassay.Results The expression levels of TIMP-1(95.36±19.67)μg/L VS(80.15±15.31)μg/L and TGF-β1(76.81±9.27)pg/mL VS(42.34±5.03)pg/mL in case group were significantly higher than those in control group,while the expression of HIF-1α(3.97±0.03)pg/mL VS(4.98±0.16)pg/mL was significantly lower(P<0.05).Compared with those in patients with mild knee osteoarthritis,the expression levels of TGF-β1 and HIF-1αwere significantly higher in patients with moderate knee osteoarthritis,while the expression of TIMP-l was significantly lower(P<0.05).Compared with those in patients with moderate knee osteoarthritis,the expression levels of TGF-β1 and HIF-1αwere significantly higher in patients with severe knee osteoarthritis,while the expression of TIMP-1 was significantly lower(P<0.05).With the aggravation of acute knee osteoarthritis,the expression levels of TGF-β1 and HIF-1αincreased,while the expression of TIMP-1 decreased(P<0.05).The diagnostic value of the combination of TIMP-1,TGF-β1 and HIF-1αwas significantly higher than that of a single detection(P<0.05).Taking the expression levels of TIMP-1,TGF-β1,and HIF-1αcorresponding to the maximum Youden index,the cut-off value of them was 689.35,5.67 and 0.42,respectively.TIMP-1 level was negatively correlated with that of TGF-β1(r=-4.794,P=0.030),which was positively correlated with HIF-1αlevel(r=5.169,P=0.024).TGF-β1 level was negatively correlated
