支撑喉镜鼻镜及纤维喉镜联合地塞米松治疗声带息肉的疗效

摘　　要：目的分析支撑喉镜鼻镜下及纤维喉镜下手术联合激素地塞米松治疗声带息肉对患者嗓音功能的影响。方法选取东莞市桥头医院于2017年2月~2020年2月收治的53例声带息肉患者作为主要研究对象,按数字表法的分组方式,将53例患者分为对照组与观察组,对照组(26例)实施支撑喉镜鼻镜下及纤维喉镜下手术,观察组(27例)在对照组的基础上联合激素地塞米松进行治疗,对两组患者治疗前后的嗓音学指标[基频微扰(Jitter)、振幅微扰(Simmer)、嗓音障碍指数(VHI)]、中性粒细胞水平及其临床指标(声带充血、水肿症状消退时间)进行比较。结果治疗后,两组Jitter以及Simmer值、VHI评分均低于治疗前,血清中性粒细胞水平低于治疗前,且观察组患者的Jitter以及Simmer值、VHI评分明显低于对照组,血清中性粒细胞水平低于对照组;与对照组相比,观察组声带充血及水肿症状消退时间较短(P<0.05)。结论采用支撑喉镜鼻镜下及纤维喉镜下手术联合激素地塞米松的方式对声带息肉患者进行治疗,能有效促进患者嗓音功能的恢复,控制声带感染,加快声带息肉相关症状消退进程。Objective To analyze the effect of nasal and fibrous laryngoscopy combined with dexamethasone treatment on vocal cord polyp on voice function.Methods 53 patients with vocal cord polyp admitted to Qiaotou Hospital from February 2017 to February 2020 were selected as the main research object.According to the grouping method of number table,53 patients were divided into control group and observation group.The control group(26 cases)received support laryngoscope nasal surgery and fiber laryngoscope surgery.Observation group(27 cases)in the control group on the basis of combined hormone dexamethasone treatment,before and after treatment in patients with two groups of voice learning index[fundamental frequency perturbation(Jitter)and amplitude perturbation(Simmer),voice disorders index(VHI)],neutrophils and their clinical indicators(vocal symptoms of congestion,edema,fade time).Results After treatment,Jitter,Simmer value and VHI score in both groups were lower than before,and serum neutrophil level was lower than before,and Jitter,Simmer value and VHI score in observation group were significantly lower than control group,and serum neutrophil level was lower than control group.Compared with the control group,the symptoms of vocal cord congestion and edema in the observation group subsided in a shorter time(P<0.05).Conclusion The treatment of patients with vocal cord polyp by means of supporting laryngoscope under nasal scope and fibrolaryngoscope combined with dexamethasone can effectively promote the recovery of vocal cord function,control vocal cord infection,and accelerate the regression process of vocal cord polypo-related symptoms.
