摘 要:目的探讨无创耳模矫治器治疗西北地区小儿先天性耳廓畸形的矫治效果,以期推广临床应用。方法分析2020年1月至2021年9月期间使用EarWell耳模矫治器治疗先天性耳廓畸形患儿,定期随访,矫治前后对比照片由耳科医师及患儿家长共同评价疗效。结果收集57例(91耳)先天性耳廓畸形患儿,先天性耳廓结构畸形6耳(6.59%),先天性耳廓形态畸形85耳(93.41%)。先天性耳廓形态畸形中环缩耳24耳(28.23%)、招风耳12耳(14.12%)和Stahl's耳11耳(12.94%)最常见。开始治疗时间为出生3~98 d,治疗疗程4~10周,无严重并发症发生。EarWell耳模矫治器治疗总有效率耳科医师评估为95.6%,患儿家属评估为97.8%(结果为优或良)。按开始治疗年龄分层评价疗效,开始治疗年龄<2周、2~6周、>6周耳科医师评估总有效率分别为100%、96.08%和50%。结论采用EarWell耳模矫治器治疗小儿先天性耳廓畸形疗效显著,尤其耳廓形态畸形,成功率高,疗效取决于耳廓畸形严重程度及开始矫治时间。因此,此种非手术治疗纠正患儿先天性耳形态畸形、轻度耳廓结构畸形方法值得推广应用。Objective To observe the efficiency of ear correction model appliances in the treatment of the infants with congenital ear abnormalities in Northwest China.Methods A review of a consecutive series of infants treated with the EarWell ear correction model from January 2020 to September 2021 was undertaken.We keep regular follow-up during the modeling period.The treatment efficiency was judged by the doctors and patients.Results In 57 babies,altogether 91 ears were treated,including 6 ears of ear malformation(6.59%)and 85 ears of ear deformation(93.41%).The helical rim abnormalities were 24 ears(28.23%),prominent ear were 12 ears(14.12%)and Stahl's ear were 11 ears(12.94%)were the most common ear deformations.The starting time of treatment was from 3 days to 98 days after birth,and all the children successfully completed the treatment,and the treatment course was from 4 weeks to 10 weeks,with no serious complications.After using the Earwell ear modeling,treatment success rate(very satisfied or satisfied)was 95.6%by doctors and 97.8%by patients.Efficacy was evaluated by age of treatment initiation,treatment success rate by doctors at<2 weeks,2-6 weeks,and>6 weeks were 100%,96.08%,and 50%respectively.Conclusions EarWell ear correction system has a high success rate in the treatment of auricular deformations and mild auricle malformations,depending on the severity of deformations and the initiation of treatment time.Therefore,this non-surgical treatment correcting congenital ear abnormalities in infants is worth popularization and application.
