摘 要:目的 分析甘肃省靖远县两个家庭聚集性新型冠状病毒肺炎病例的流行病学特征,查明传播链和新型冠状病毒潜伏期的传播特征。方法 采用现场流行病学调查方法,收集流行病学相关资料,分析潜伏期、发病前后和不同亲缘关系密切接触续发率。结果 病例1于2020年1月25日出现症状,1月30日确诊,病例1潜伏期,即症状出现前5 d和病例2有接触史;排除其他暴露因素,病例2于2020年1月31日出现症状,2月6日确诊。病例1在潜伏期共造成发病4例(包括病例2)。病例1的密切接触者中发病9例,病例2的密切接触者中发病1例;病例1密切接触者续发率为29.03%(9/31),病例2密切接触者续发率2.04%(1/49),差异有统计学意义(χ^(2)=12.373,P<0.05)。与病例接触时间在8~12 h/d的续发病例6例,与病例接触时间在2~5 h的续发率为5.63%(4/71),不同接触时间续发率差异有统计学意义(χ^(2)=21.909,P<0.05)。2例病例密切接触者中一级亲属续发病例4例,二级亲属续发病例1例,三级亲属续发率为14.29%(3/21),非亲属续发率为4.44%(2/45);不同级别亲属续发率差异有统计学意义(χ^(2)=13.288,P<0.05)。结论 新型冠状病毒肺炎病例在潜伏期已具有传染性,对密切接触者的定义应提前至病例出现症状前4~5 d。Objective To analyze the epidemiological characteristics of the patients of two family clustering of novel coronavirus infection in Jingyuan country of Gansu province,so as to find out the chain of transmission and the transmission characteristics in the incubation period of novel coronavirus infection.Methods The epidemiological data was collected with field epidemiological investigation method for analyzing the secondary attack rates of the close contactors in the incubation period,the time before and after the disease onset and in the persons who had different relative relationships with the patient.Results On January 252020,the patient 1 developed symptoms and was confirmed on January 302020,and contacted with patient 2 within the incubation period that was 5 days before the occurrence of symptoms.The patient 2 developed symptoms on January 312020 and was confirmed on February 62020.The exposure factors were excluded.There were 4 patients(including patient 2)caused by patient 1 within the incubation period.There were 9 patients occurred in the close contactors of patient 1,with the secondary attack rate of 29.03%(9/31),and 1 patients occurred in the close contactors of patient 2,with the secondary attack rate of 2.04%(1/49),and the difference was statistically significant(χ^(2)=12.373,P<0.05).There were 6 secondary cases contacted with patient during 8-12 h/d,and the secondary attack rate was 5.63%(4/71)during 2-5 h/d of the contacting time with the patient.The difference was statistically significant(χ^(2)=21.909,P<0.05)in the secondary attack rates in different contact time.There were 4 patients in the first-degree relatives,1 patient in the seconddegree relatives.The recurrence rate was 14.29%(3/21)in third-degree relatives,and 4.44%(2/45)in non-relatives among the close contactors with the 2 patients.There was statistical difference(χ^(2)=13.288,P<0.05)in the secondary attack rates among the different degree relatives..Conclusions There is infectivity in the patients with novel coronavirus infection du
