摘　　要：目的探讨中青年卒中患者社会疏离感的成因,为进一步拟定中青年脑卒中患者社会疏离感的干预措施提供依据。方法使用一般疏离感量表调查并选取存在社会疏离感的15例中青年脑卒中患者进行半结构式访谈,运用现象学方法分析访谈资料并提炼主题。结果通过对15例中青年卒中患者的访谈资料进行分析,共截取有效的479个语义片段,提炼出2个主题及6个亚主题,分别为主动疏离(自我形象改变、角色改变、自我负担、消极情绪体验)、被动疏离(社会支持缺乏和重返社会困难)。结论中青年脑卒中患者的社会疏离感的成因涉及患者个人、家庭、医疗康复、社会支持多个方面,需加强多方协作,帮助患者正确进行康复锻炼,减轻负性情绪,提高其社会参与度与社会融入感,减少社会疏离感的产生。Objective To investigate the causes of social alienation in young and middle-aged stroke patients so as to provide a basis for further development of interventions in social alienation.Methods General Alienation Scale was used among the young and middle-aged stroke patients with social alienation.Semi-structured interviews were conducted to analyse the acquired data,followed by abstracting themes by using phenomenological methods.ResultsA total of 479 valid semantic fragments were extracted from the data of interviews with 15 young and middle-aged stroke patients.Two themes with 6 sub-themes were abstracted.They were an active alienation which included change of self-image,change of roles,self-burden and negative emotional experiences,and a passive alienation that included lack of social support and difficulties in social reintegration.Conclusions Social alienation in young and middle-aged stroke patients attributes to multiple factors in personality,family,medical rehabilitation and social support.It is necessary to strengthen the collaboration from multiple aspects to help patients to carry out rehabilitation proper exercises,minimise negative emotions and improve their social participation and social integration,ultimately reducing the social alienation.
