无痛分娩联合人文护理对足月分娩产妇产程、产道裂伤的影响

摘　　要：目的:研究无痛分娩联合人文护理对足月分娩产妇产程、产道裂伤的影响。方法:纳入2021年3月至2022年3月本院收治的100例足月待产产妇为研究对象,用随机数字表法分为对照组和研究组,各50例。对照组给予常规自然分娩及产后护理,研究组则在对照组基础上给予无痛分娩联合人文护理。比较分析两组产妇入院时和一个月后焦虑自评量表(Self-rating anxiety scale,SAS)评分,记录比较两组新生儿1min和5min新生儿Apgar评分,分娩后产妇并发症发生率、各产程时间、产后2h出血量及产道裂伤恢复时间,并对比两组产妇产道裂伤修复满意度以评价干预效果。结果:产后一个月时,两组产妇SAS评分均明显降低,且研究组低于对照组,差异均有统计学意义(均P<0.05)。研究组各产程时间、产后2h出血量、并发症发生率及产道裂伤恢复时间均明显低于对照组,差异均有统计学意义(均P<0.05)。研究组新生儿的1min新生儿Apgar评分、5min新生儿Apgar评分和产后产道恢复满意度均高于对照组,差异均有统计学意义(均P<0.05)。结论:无痛分娩联合人文护理可有效缩短产程和产道裂伤恢复时间,降低产妇负面心理,减少新生儿窒息风险,提高产道裂伤恢复效果,值得应用。Objective:To study the effect of painless delivery combined with humanistic nursing on the labor process and birth canal laceration of full-term parturient.Methods:A total of 100 full-term pregnant who admitted to our hospital from March 2021 to March 2022 were selected as research object,and they were randomly divided into the control group and the research group,50 cases in each group.The control group was given routine natural delivery and postpartum care,while the research group was given painless delivery combined with humanistic care on the basis of the control group.The self rating anxiety scale(SAS)scores at admission and one month after delivery,Apgar scores at 1 min and 5 min for newborns in the two groups,the occurrence of complications after delivery,the time of each birth process,the amount of bleeding in 2 hours after delivery,the recovery time of laceration of the birth canal,and satisfaction with repair of laceration of the birth canal between the two groups were compared and analyzed to evaluate the intervention effect.Results:One month after delivery,the SAS scores of the two groups were significantly lower,and the research group was lower than that of the control group,these differences were statistically significant(all P<0.05).The time of each birth process,the amount of bleeding in 2 hours after delivery,the incidence of postpartum complications and the recovery time of laceration of the birth canal in the research group were significantly lower than those in the control group(all P<0.05).The one-minutes Apgar score,five-minutes Apgar score and postpartum satisfaction of the newborns in the research group were significantly higher than those in the control group(all P<0.05).Conclusion:Painless delivery combined with humanistic nursing can effectively shorten the labor process and recovery time of birth canal laceration,reduce maternal negative psychology,reduce the risk of neonatal asphyxia,and improve the recovery effect of birth canal laceration,which is worthy of reference.
