摘 要:目的明确LED红光联合胰岛素外用对糖尿病大鼠创面愈合的影响及其初步机制。方法将40只雄性SD大鼠随机分为对照组、红光组、胰岛素组、红光联合胰岛素组,高脂饲养后腹腔注射链脲佐菌素(streptozotocin,STZ)进行糖尿病造模,成功后在大鼠的背部制造4个全层皮肤缺损创面,每两天对红光联合胰岛素组及红光组大鼠的创面进行20J/cm^(2)的红光照射,胰岛素组及红光联合胰岛素组大鼠创面滴加0.5U短效胰岛素。在第0、7、10、14、21天,麻醉大鼠后拍摄创面照片并用image J软件计算大鼠创面愈合率;在第10、14天在各组随机选择两只大鼠,取创缘组织固定后进行HE染色观察组织新生血管情况以及用免疫荧光法检测VEGF、CD34表达情况,用免疫组化法检测p-Akt、Akt、HIF-1α表达情况。结果在各个时间点,红光联合胰岛素组创面愈合率均高于其他组,差异有统计学意义(P<0.05);第10、14天,创面组织HE染色显示,红光联合胰岛素组创面组织内见大量新生毛细血管,胰岛素组及红光组新生毛细血管少于红光联合胰岛素组,对照组新生血管最少;用免疫荧光法检测CD34、VEGF表达情况,红光联合胰岛素组CD34和VEGF表达量高于其他组,而红光组和胰岛素组表达高于对照组;红光联合胰岛素组p-Akt、HIF-1α表达高于其他组,而红光组和胰岛素组p-Akt、HIF-1α表达高于对照组;各组Akt表达量比较,差异无统计学意义。结论LED红光和胰岛素联合应用介导PI_(3)K/Akt/HIF-1α/VEGF信号通路促进糖尿病大鼠创面愈合。Objective To study the effect of 640nm LED red light irradiation on promoting wound healing in diabetic rats and its mechanism.Methods Forty male SD rats were randomly divided into control group,red light group,insulin group,and red light combined with insulin group.After high-fat feeding,intraperitoneal injection of streptozotocin(STZ)was used to establish a diabetic model,then four full-thickness skin defect wounds were created on the back of the rat.The wounds of the rats in the red light combined with insulin group and the red light group were irradiated with 20J/cm^(2) of red light every 2days,and the wounds of the rats in the insulin group and the red light combined with insulin group were dripped with 0.5U short-acting insulin.On the 0th,7th,10th,14th,and 21st days of the observation,after the rats were anesthetized,the wound healing rate of the rats was calculated using image J software after taking pictures.On the 10th and 14th day of the observation,2 rats were randomly selected from each group,and the wound edge tissue was taken.After fixation,HE staining was performed to observe the neovascularization of the tissue,and the expression of VEGF and CD34 was detected by immunofluorescence,and the expression of p-Akt,Akt and HIF-1αwas detected by immunohistochemistry.Results At each time point,the wound healing rate of the red light combined with insulin group was higher than that of the other groups,and the difference was statistically significant(P<0.05);on the 10th and 14th day of observation,hematoxylin-eosin staining of the wound tissue showed that a large number of new capillaries were seen in the wound tissue of the red light combined with insulin group,and the insulin group and the red light group had lots of new capillaries,but less in the red light combined with insulin group,the control group had the least new blood vessels;the expression of CD34 and VEGF was detected by immunofluorescence method,the expression of CD34 and VEGF in the red light combined with insulin group was higher than that i
