摘　　要：目的分析经阴道三维超声检查对不同程度及类型宫腔粘连的诊断意义。方法收集疑似宫腔粘连患者329例,所有患者均行经阴道三维超声检查,以宫腔镜检查为诊断金标准。分析经阴道三维超声对不同程度及类型宫腔粘连的诊断价值。结果经阴道三维超声检查结果显示,宫腔粘连轻度115例(34.95%),中度116例(35.26%),重度98例(29.79%);宫腔镜检查结果显示,宫腔粘连轻度139例(42.25%),中度97例(29.48%),重度93例(28.27%)。两种检查方式对不同程度宫腔粘连的诊断率比较差异无统计学意义(Z=-1.443,P>0.05)。经阴道三维超声检查结果显示,周围型119例(36.17%),中央型92例(27.96%),混合型118例(35.87%);宫腔镜结果显示,周围型103例(31.31%),中央型105例(31.91%),混合型121例(36.78%)。两种检查方式对不同类型宫腔粘连的诊断率比较,差异无统计学意义(Z=-0.872,P>0.05)。结论经阴道三维超声对不同类型及程度的宫腔粘连的诊断有较高的准确性,能够从多个维度观察宫腔及内膜情况,直观全面地显示粘连的部位、范围,值得临床应用。Objective To analyze the diagnostic significance of transvaginal three-dimensional ultrasonography in different degrees and types of uterine adhesions.Methods A total of 329 patients with suspected uterine adhesions were collected.All patients underwent transvaginal three-dimensional ultrasonography.Hysteroscopy was used as the gold standard for diagnosis.To analyze the value of transvaginal three-dimensional ultrasound in the diagnosis of different degrees and types of uterine adhesions.Results The results of transvaginal three-dimensional ultrasonography showed that 115 cases(34.95%)of uterine adhesions were mild,116 cases(35.26%)moderate,and 98 cases(29.79%)severe.The results of hysteroscopy showed that 139 cases(42.25%)of uterine adhesion were mild,97 cases(29.48%)were moderate,and 93 cases(28.27%)were severe.There was no significant difference in the detection rates of uterine adhesions of different degrees between the two methods(Z=-1.443,P>0.05).The results of transvaginal three-dimensional ultrasonography showed:119 cases(36.17%)of peripheral type,92 cases(27.96%)of central type,118 cases(35.87%)of mixed type.Hysteroscopic results showed that 103 cases(31.31%)of peripheral type,105 cases(31.91%)of central type,121 cases(36.78%)of mixed type.There was no statistical significance in the diagnostic rates of different types of uterine adhesions between the two methods(Z=-0.872,P>0.05).Conclusion Transvaginal three-dimensional ultrasound has high accuracy in the diagnosis of different types and degrees of uterine adhesions.Which can observe uterine cavity and endometrium from multiple dimensions,intuitively and comprehensively show the position and scope of adhesions which is worthy of clinical application.
