多囊卵巢综合征患者三维多普勒超声下卵巢血流动力学与血清性激素水平的相关性分析

摘　　要：目的探讨三维多普勒超声测量多囊卵巢综合征(PCOS)患者卵巢内血流分布及血流指标,评价其对PCOS的诊断价值及与血清性激素水平的相关性。方法选择行三维多普勒超声检查的PCOS患者80例为研究组,同时选择与患者年龄匹配的健康体检人群80例为对照组。全部研究对象月经干净后第3~8天进行多普勒超声检查,并空腹抽血检测性激素水平。结果研究组血管化指数(VI)、血流指数(FI)和血管化-血流指数(VFI)均高于健康对照组(P<0.05);研究组的总睾酮(T)和促黄体生成素(LH)高于对照组,而卵泡刺激素(FSH)低于对照组(P<0.05)。研究组VI、FI和VFI与LH呈正相关关系,VI、FI和VFI与FSH呈负相关关系,同时VFI与T呈正相关关系(P<0.05);VFI诊断PCOS的敏感度和特异度分别为72.8%和81.4%,其受试者工作特征曲线(ROC)的曲线下面积(AUC)为0.81,截断值为2.89。结论三维多普勒超声能够观察和定量评价卵巢血流分布,具有较高的诊断价值,可为临床辅助诊断PCOS提供参考。Objective To investigate the measurement of blood flow distribution and blood flow indexes in ovary of polycystic ovary syndrome(PCOS)patients by three-dimensional Doppler ultrasound,and to evaluate its diagnostic value for PCOS and its correlation with serum sex hormone levels.Methods Eighty patients with PCOS who underwent three-dimensional Doppler ultrasonography were selected as the study group,and 80 healthy people matching the age of the patients were selected as the control group.All subjects underwent Doppler ultrasonography 3-8 days after clean menstruation,and fasting blood was drawn to detect sex hormone levels.Results The vascularization index(VI),flow index(FI)and vascularization flow index(VFI)of the study group were higher than those of the healthy control group(P<0.05).The total testosterone(T)and luteinizing hormone(LH)in the study group were higher than those in the control group,but the follicle stimulating hormone(FSH)was lower than those in the control group(P<0.05).In the study group,VI,FI and VFI were positively correlated with LH,VI,FI and VFI were negatively correlated with FSH,and VFI was positively correlated with T(P<0.05).The sensitivity and specifi city of VFI in the diagnosis of PCOS were 72.8%and 81.4%,respectively.The area under receiver operating characteristic curve(ROC)curve(AUC)was 0.81,and the cutoff value was 2.89.Conclusion Three-dimensional Doppler ultrasound can observe and quantitatively evaluate the ovarian blood flow distribution,which has high diagnostic value,and can provide reference for clinical diagnosis of PCOS.
