自我同情对不孕症IVF-ET反复种植失败患者再次妊娠结局的影响

摘　　要：目的探讨不孕症体外授精-胚胎移植(IVF-ET)反复种植失败患者自我同情对再次妊娠结局的影响,为临床制定措施改善患者再次妊娠结局提供依据。方法纳入女性不孕症IVF-ET反复种植失败患者进行调查,采用自我同情量表评估自我同情水平,给予患者第4次IVF-ET治疗,IVF-ET治疗后4周,通过超声评估患者再次妊娠结局并将其分为妊娠成功组和妊娠失败组,比较妊娠成功组、妊娠失败组自我同情水平及一般资料,重点分析自我同情对不孕症IVF-ET反复种植失败患者再次妊娠结局的影响。结果本研究共纳入129例不孕症IVF-ET反复种植失败患者,发放一般资料调查表、自我同情量表、中文版Herth希望量表及领悟社会支持量表各129份,回收有效问卷124份,有效回收率为96.12%。第4次IVF-ET治疗后4周,超声检查结果显示,124例患者再次妊娠成功64例(51.61%),再次妊娠失败60例(47.39%)。妊娠失败组自我同情量表、中文版Herth希望量表及领悟社会支持量表评分均低于妊娠成功组,差异有统计学意义(P<0.01)。经点二列相关性分析,不孕症IVF-ET反复种植失败患者自我同情量表评分与再次妊娠结局呈负相关(r=-0.724,P<0.01)。经Logistic回归分析显示,自我同情、希望水平、领悟社会支持是不孕症IVF-ET反复种植失败患者再次妊娠结局的影响因素(P<0.05)。结论不孕症IVF-ET反复种植失败患者自我同情水平较低,且自我同情可对患者再次妊娠结局造成影响。Objective To explore the influence of self-compassion on the repregnancy outcome of infertile patients with repeated implantation failure of in vitro fertilization embryo transfer(IVF-ET)in order to provide bases for making clinical measures to improve the repregnancy outcome.Methods Female infertility patients with repeated implantation failure of IVF-ET were included for investigation,self compassion levels were evaluated with the Self-Compassion Scale(SCS),the 4th IVF-ET treatments were given to the patients,in the 4th week after IVF-ET treatment they were assigned to successful and failed pregnancy group according to the repregnancy outcome evaluated by ultrasound,self-compassion levels and general data were compared between two groups,and the effects of self-compassion on the repregnancy outcome of infertility patients with repeated implantation failure of IVF-ET analyzed.Results A total of 129 infertility patients with repeated implantation failure of IVF-ET were enrolled in this study,129 copies of general information questionnaires,Self-Compassion Scale(SCS),Herth Hope Index(HHI)and Perceived Social Support Scale(PSSS)were respectively distributed,124 valid questionnaires were retrieved,and effective retrieving rate was 96.12%.In the 4th week after the 4th IVF-ET treatment ultrasonic examination results showed that of 124 patients 64(51.61%)had successful repregnancies and 60(47.39%)failed.Scores on the SCS,HHI and PSSS were lower in failed than in successful pregnancy group,differences had statistical significance(P<0.01).Point binary correlation analysis showed that score on the SCS was negatively correlated with repregnancy outcome(r=-0.724,P<0.01).Logistic regression analysis showed that self-compassion,hope levels and perceived social support were factors influencing the repregnancy outcome of infertility patients with repeated IVF-ET implantation failure(P<0.05).Conclusion Infertility patients with repeated implantation failure of IVF-ET have lower levels of self-compassion and self-compassion could
