摘　　要：目的 观察经食管超声心动图(TEE)独立引导经股静脉途径封堵治疗儿童动脉导管未闭(PDA)的价值。方法 收集25例接受TEE独立引导下经股静脉途径封堵治疗的PDA患儿(TEE组),并纳入接受X线引导下介入封堵治疗的70例PDA患儿(对照组),对比2组患儿一般资料、手术情况及随访资料,评估TEE引导下经股静脉途径封堵的疗效。结果 2组治疗效果均满意,封堵器位置均正常,周围无残余分流、无大血管梗阻。TEE组手术时间为28~61 min,对照组为28~62 min。组间患儿年龄、手术时间及住院时间差异均无统计学意义(P均>0.05)。治疗后1、3及6个月,2组封堵器位置均正常、动脉水平无残余分流、无心律失常;均未见明显并发症,无死亡病例。结论 TEE用于独立引导经股静脉途径封堵治疗儿童PDA安全、有效。Objective To observe the value of transesophageal echocardiography(TEE)alone for guiding transcatheter closure of patent ductus arteriosus(PDA)via femoral vein in children.Methods Twenty-five children who underwent TEE-guided transcatheter closure of PDA via femoral vein(TEE group)and 70 children who underwent X-ray guided transcatheter closure of PDA(control group)were enrolled.The general data,surgical data and follow-up data were comparatively observed between groups,and the efficacy of TEE for guiding transcatheter closure of PDA via femoral vein was analyzed.Results The therapeutic effect was satisfactory in both groups.The position of occluders were all normal,without residual shunts around nor large vessel obstruction.The operation time of TEE group ranged from 28—61 min,while of control group was 28—62 min.No significant difference of age,operation time nor hospital stay was found between groups(all P>0.05).One,3 and 6 months after closure,the position of occluders in both groups remained normal,without arterial residual shunts,arrhythmias,complications nor death.Conclusion TEE alone was valuable for guiding transcatheter closure of PDA via femoral vein in children.
