摘　　要：目的:探讨经阴道三维超声在宫内节育器(IUD)取出困难患者中的诊断价值。方法:选取2018年7月—2022年6月嘉应学院医学院附属医院收治的IUD取出困难患者80例为观察对象,患者均行阴道三维、二维超声检查,且经宫腔镜手术,以宫腔镜结果为标准,比较二维、三维超声诊断准确性及准确率。结果:80例患者中,发生IUD嵌顿37例,断裂残留25例,转位10例,宫腔粘连4例,黏膜下肌瘤3例,子宫畸形1例。二维超声诊断准确率低于经阴道三维超声,差异有统计学意义(P<0.001)。结论:在IUD取出困难患者中,术前给予经阴道三维超声检查,利用自由解剖成像等技术,可准确发现IUD取出困难的原因,提升诊断准确性,有助于手术方案的完整。Objective:To investigate the diagnostic value of transvaginal three-dimensional ultrasound in patients with difficulty in removing intrauterine device(IUD).Methods:Eighty patients with difficulty in removing IUD admitted to the Affiliated Hospital of Jiaying University Medical College from July 2018 to June 2022 were selected as the observation objects.All patients underwent three-dimensional and two-dimensional vaginal ultrasound and hysteroscopic surgery.The diagnostic accuracy and accuracy of 2D and 3D ultrasound were compared based on hysteroscopy results.Results:Among the 80 patients,37 had IUD incarceration,25 had rupture residual,10 had translocation,4 had intrauterine adhesions,3 had submucosal myoma,and 1 had uterine malformation.The diagnostic accuracy of two-dimensional ultrasound was lower than that of transvaginal three-dimensional ultrasound,the difference was statistically significant(P<0.001).Conclusion:In patients with difficulty in removing IUD,preoperative transvaginal three-dimensional ultrasound,free anatomical imaging and other techniques can accurately discover the causes of difficulty in removing IUD,improve the diagnostic accuracy and contribute to the completeness of the surgical program.
