miRNA-21及相关临床指标在狼疮性肾炎肾衰竭患者中的表达及其意义

摘　　要：【目的】探讨微小RNA-21(miRNA-21)及相关临床指标在狼疮性肾炎(LN)肾衰竭患者中的表达变化及其意义。【方法】本院收治的85例LN患者,将其中存在肾衰竭的患者作为观察组(n=19),余为对照组(n=66)。检测并比较两组患者血清中miRNA-21的表达及相关指标的差异,应用受试者工作特征(ROC)曲线分析miRNA-21诊断LN肾衰竭的价值,采用多因素Logistic回归分析影响LN肾衰竭的相关因素。【结果】观察组估测肾小球滤过率(eGFR)、补体C3水平低于对照组,但系统性红斑狼疮(SLE)疾病活动指数(SLEDAI)-2000评分、血尿素氮(BUN)、血肌酐(Scr)、血沉及miRNA-21水平高于对照组患者,且差异有统计学意义(P<0.05);SLEDAI-2000评分、血沉及miRNA-21是LN患者肾衰竭的危险因素,其优势比(OR)(95%CI)分别为3.137(1.775~5.543)、4.894(2.472~9.687)及1.227(1.091~1.531);miRNA-21诊断LN肾衰竭的曲线下面积(AUC)(95%CI)为0.78(0.65~0.91)。【结论】LN肾衰竭患者中miRNA-21、SLEDAI-2000评分及血沉升高,其中miRNA-21是可能诊断LN肾衰竭的有益指标。【Objective】To analyze the expression and value of miRNA-21 with related clinical indicators in lupus nephritis(LN)with renal failure.【Methods】A total of of 85 patients with LN were selected in our hospital.Patients with renal failure were included in the observation group(n=19),the others were grouped as the control group(n=66).Levels of miRNA-21 and its related clinical indicators in serum were detected and compared between two groups,then receiver operating characteristic(ROC)curve analysis was used to evaluate the value of miRNA-21 in renal failure.Meanwhile,the influencing factors of renal failure were analyzed by using logistic regression analysis.【Results】The estimate glomerular filtration rate(eGFR)and complement C3 levels in the observation group were lower than those in the control group,while the SLE disease activity index(SLEDAI)-2000 score,blood urea nitrogen(BUN),serum creatinine(Scr),erythrocyte sedimentation rate(ESR)and miRNA-21 in the observation group were significantly higher in the control group.The difference between the two groups was statistically significant(P<0.05).SLEDAI-2000 score,ESR and miRNA-21 were closely related to renal failure in LN,whose odds ratios(OR)(95%CI)were 3.137(1.775-5.543),4.894(2.472-9.687)and 1.227(1.091-1.531),respectively.The area under the curve(AUC)(95%CI)of miRNA-21 in diagnosis of renal failure with LN was 0.78(0.65-0.91).【Conclusion】The level of miRNA-21,SLEDAI-2000 score and ESR are higher in LN with renal failure,and miRNA-21 may be a useful indicator for diagnosing renal failure in LN.
