锝亚甲基二磷酸盐、甲氨蝶呤、艾拉莫德联合应用对类风湿关节炎IL-6、TNF-α水平的影响

摘　　要：目的探析类风湿关节炎患者接受锝亚甲基二磷酸盐、甲氨蝶呤、艾拉莫德联合治疗的临床疗效,及对白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)表达水平的影响。方法随机选择2021年1—12月兰陵县人民医院收治的200例类风湿关节炎患者作为研究对象,按随机数表法均分为两组(对照组、观察组,各100例),对照组采用甲氨蝶呤+艾拉莫德治疗,观察组采用锝亚甲基二磷酸盐+甲氨蝶呤+艾拉莫德治疗。比较两组治疗前后的炎性因子水平、不良事件、临床疗效。结果治疗后3个月,观察组IL-6(2.47±0.87)ng/mL、TNF-α(4.22±1.06)ng/mL、DAS28评分(2.12±0.23)分优于对照组,差异有统计学意义(P<0.05);观察组、对照组总有效率分别为98.00%、90.00%,组间比较,差异有统计学意义(χ^(2)=5.674,P<0.05)。结论锝亚甲基二磷酸盐、甲氨蝶呤、艾拉莫德联合治疗在类风湿关节炎中能达到较显著的用药效果,能改善炎性因子水平,促进关节肿痛、晨僵等症状的恢复,且用药安全度高。Objective To investigate the clinical efficacy of the combination of technetium methylene diphosphonate,methotrexate,and iguratimod in patients with rheumatoid arthritis and the effects on the expression levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α).Methods Two hundred patients with rheumatoid arthritis admitted to Lanling County People's Hospital from January to December 2021 were random selected as study subjects and divided equally into two groups(control group and observation group,100 cases each)according to the random number table method.The control group was treated with methotrexate+iguratimod,and the observation group was treated with technetium methylene diphosphonate+methotrexate+iguratimod.The levels of inflammatory factors,adverse events,and clinical efficacy before and after treatment were compared between the two groups.Results Three months after treatment,IL-6(2.47±0.87)ng/mL,TNF-α(4.22±1.06)ng/mL,and DAS28 score(2.12±0.23)points were better in the observation group than in the control group,the difference was statistically significant(P<0.05).The total effective rate was 98.00%and 90.00%in the observation and control groups,respectively,and the difference was statistically significant when compared between groups(χ^(2)=5.674,P<0.05).Conclusion The combination of technetium methylene diphosphonate,methotrexate and iguratimod can achieve more significant medication effect in rheumatoid arthritis,improve the level of inflammatory factors,promote the recovery of joint swelling and pain,morning stiffness and other symptoms,and the medication safety is high.
