电子毛发镜观察雄激素性秃发的临床研究

摘　　要：目的:探讨应用电子毛发镜临床观察雄激素性秃发的方法及价值。方法:选取2023年2—3月在上海市皮肤病医院毛发诊疗中心就诊的174例雄激素性秃发患者为研究组,另选取27例健康志愿者为对照组,观察并比较2组受试者的毛发镜征象及相关临床资料。结果:与健康志愿者相比,雄激素性秃发患者的平均毛干直径减小,毳毛增多、终毛减少,只有1根毛发的毛囊单位占比增高、有2根毛发的毛囊单位占比降低,黄点增多,针尖样白点、蜂窝状色素和白色鳞屑均增多,差异均具有统计学意义(均P<0.05)。结论:在临床诊断中,平均毛干直径,毳毛和终毛比例,毛囊中毛干数量的变化,黄点、针尖样白点、蜂窝状色素沉着和白色鳞屑的阳性率均可用作判断患者是否罹患雄激素性秃发的参考指标。Objective:To explore the method and value of the application of trichoscopy in clinical observation of androgenetic alopecia(AGA).Methods:A total of 174 patients with AGA who visited the Hair Diagnosis and Treatment Center of Shanghai Skin Disease Hospital from February to March,2023 were collected as the experimental group,and another 27 healthy volunteers were chosen as the control group.The trichoscopic signs and related clinical data of the two groups were observed and compared.Results:The average hair stem diameter and terminal hair decreased,while vellus hair,the proportion of hair follicle units in a single and two hair,yellow spots,needle like white spots,honeycomb like pigments and white scales increased in patients with AGA(all P<0.05).Conclusion:In clinical diagnosis,the average hair stem diameter,ratio of vellus hair to terminal hair,changes in the number of hair stems in hair follicles,positive rates of yellow spots and needle like white spots,honeycomb like pigmentation,and white scales can be used as indicators to determine whether a patient has AGA.
