摘　　要：目的系统性评价分娩计划书对初产妇分娩结局的影响。方法计算机检索中国知网、中国生物医学、维普、PubMed、Cochrane Library、Web of Science等数据库,搜集关于分娩计划书对初产妇分娩结局影响的类实验研究及随机对照研究,检索时间从建库至2021年4月。由2名研究人员独立筛选文献、提取资料并评价文献质量后,采用RevMan 5.3软件进行meta分析。结果本研究共纳入12篇文献。meta分析结果显示,分娩计划书可降低剖宫产率[OR=0.49,95%CI(0.42,0.58),P<0.00001],降低会阴侧切率[OR=0.73,95%CI(0.59,0.90),P=0.003],提高分娩满意度[OR=4.05,95%CI(2.26,7.28),P<0.00001]。结论分娩计划书可降低初产妇的剖宫产率、会阴侧切率并提高分娩满意度,值得临床推广应用。Objective To systematically evaluate the effect of birth plan on deliver y outcomes of primiparas.Methods China National Knowledge Infrastructure,China Biology Medicine,VIP,PubMed,Cochrane Library,Web of Science and other databases were searched by computer to collect quasi-experimental studies and randomized controlled studies on the effect of birth plan on delivery outcomes of primiparas.The search time was from the establishment of the database to April 2021.Two researchers independently screened the literature,extracted data and evaluated the quality of the literature.meta-analysis was performed using RevMan 5.3 software.Results A total of 12 articles were included in this study.The results of meta-analysis showed that the birth plan could reduce the rate of cesarean section(OR=0.49,95%CI[0.42,0.58],P<0.00001),the rate of episiotomy(OR=0.73,95%CI[0.59,0.90],P=0.003),and improve delivery satisfaction(OR=4.05,95%CI[2.26,7.28],P<0.00001).Conclusion The birth plan can reduce the cesarean section rate and episiotomy rate of primiparas and improve the delivery satisfaction,which is worthy of clinical application and promotion.
