1107例宫颈上皮内瘤变阴道镜图像分析及意义

摘　　要：目的分析宫颈上皮内瘤变阴道镜图像特征,为诊断高级别鳞状上皮内病变(HSIL)提供依据。方法选取2020年12月至2023年1月于石河子大学医学院第一附属医院妇科门诊经病理诊断为宫颈上皮内瘤变的1107例患者为研究对象,按照病理结果分为低级别鳞状上皮内病变(LSIL)组(n=704)和HSIL组(n=403)。分析两组阴道镜图像特征。结果HSIL组异常血管粗镶嵌、粗点状占比高于LSIL组(P<0.05),HSIL组厚醋白上皮、醋白上皮表面隆起、醋白上皮边界锐利或卷曲状占比高于LSIL组(P<0.05)。两组碘试验阴性比较,差异无统计学意义(P>0.05)。结论阴道镜下异常血管、醋白上皮有助于宫颈上皮内瘤变的诊断。Objective To analyze the colposcopic image characteristics of cervical intraepithelial neoplasia,and to provide evidence for the diagnosis of cervical high-grade squamous intraepithelial lesion(HSIL).Methods A total of 1107 patients with cervical intraepithelial neoplasia diagnosed by pathology in the Gynecological Clinic of the First Affiliated Hospital of Shihezi University from December 2020 to January 2023 were selected as the research objects.According to the pathological results,they were divided into low-grade squamous intraepithelial lesion(LSIL)group(n=704)and HSIL group(n=403).The characteristics of colposcopy images of the two groups were analyzed.Results The proportion of coars e mosaic and coarse punctate abnormal vessels in group H SIL was higher than that in group L SIL(P<0.05).The proportion of thick acetic acid white epithelial,vinegar white epithelial surface eminence,vinegar white epithelial border sharp or curly in HSIL group was higher than that in LSIL group(P<0.05).There was no significant difference in iodine test between the two groups(P>0.05).Conclusion Abnormal blood vessels and vinegar white epithelium under colposcopy are helpful for the diagnosis of ce rvical intraepithelial neoplasia.
