双主体三步法护理模式对选择性减胎孕妇抑郁及产检依从性的影响

摘　　要：目的探讨双主体三步法护理模式在中期妊娠减胎术抑郁中的应用效果。方法选取90例实施多胎妊娠减胎术的孕妇,采用随机数字表法分为干预组和对照组,各45例,剔除随访过程中脱落病例8例,最终对照组40例和干预组42例。对照组采用常规护理,干预组在对照组基础上采用双主体三步法护理模式,即医院和家庭两个主体,通过围术期全面评估-个性化干预-延续护理的“三步法”综合管理模式进行干预。比较两组孕妇干预前后抑郁自评量表(SDS)评分、减胎术相关知识掌握度评分及干预后产检依从性。结果干预前,两组抑郁评分、减胎术相关知识掌握度比较,差异无统计学意义(P>0.05);干预后,干预组抑郁评分低于对照组,减胎术相关知识掌握度评分高于对照组,差异有统计学意义(P<0.05)。干预组的产检依从率为95.2%,明显高于对照组的80.0%,差异具有统计学意义(P<0.05)。结论双主体三步法护理促进孕妇积极心理状态,有效提高减胎术孕妇相关知识掌握水平及产检依从性。Objective To investigatetheeffect of two-subject three-step nursingmodel in depression duringpregnancy reduction.Methods 90 pregnant women who underwent multiple pregnancy reduction operation were selected and divided into intervention group and control group by random number table method,45 cases in each group,8 cases of shedding during follow-up were excluded,and 40 cases in the final control group and 42 cases in the intervention group.The control group was treated with routine nursing,and the intervention group was treated with wo-subject three-step nursing model on the basis of the control group,namely the two subjects of hospital and family.The intervention was carried out by the"three-step"comprehensive management mode of perioperative comprehensive assessment-personalized intervention-extended care.The scores of self-rating depression scale(SDS)and mastery of knowledge related to fetal reduction and the compliance of postpartum examination after intervention were compared between the two groups.Results Before intervention,there was no significant difference in depression score and knowledge of fetal reduction between the two groups(P>0.05).After the intervention,the depression score of the intervention group was lower than that of the control group,and the score of the knowledge related to fetal reduction was higher than that of the control group,with statistical significance(P<0.05).The compliance rate of the intervention group was 95.2%,significantly higher than that of the control group(80.0%),and the difference was statistically significant(P<0.05).Conclusion Two-subject three-step nursing model can promote the positive psychological state of pregnant women and effectively improve the mastery level of relevant knowledge and compliance of prenatal examination.
