摘  要:目的对难治性鼻出血患者实施低温等离子联合微填塞技术治疗,分析临床治疗效果和安全性。方法随机选取2019年1月—2022年6月启东市中医院耳鼻喉科治疗的80例难治性鼻出血患者为研究对象,并按照系统盲选法分为两组,各40例。常规组实施鼻内镜下后鼻孔填塞+鼻腔凡士林纱条填塞治疗,研究组则实施低温等离子+微填塞技术治疗。评估两组患者主观不适和鼻腔粘连情况,分析临床治疗效果。结果研究组治愈率97.50%(39/40)明显高于常规组80.00%(32/40),差异有统计学意义(χ^(2)=4.507,P<0.05)。在主观不适VAS评分方面,研究组鼻塞(1.88±2.05)分、鼻部胀痛(2.62±2.31)分、头部疼痛(2.48±1.92)分、溢泪(2.06±1.44)分、吞咽困难(0.73±0.56)分、睡眠障碍(2.04±1.83)分均低于常规组(5.20±2.18)分、(5.58±2.25)分、(5.61±2.03)分、(3.77±1.86)分、(5.18±1.39)分、(5.57±2.34)分,差异有统计学意义(P<0.05)。研究组术后鼻腔粘连发生率0.00%(0/40)较常规组17.50%(7/40)更低,差异有统计学意义(P<0.05)。结论低温等离子与微填塞技术联合应用在难治性鼻出血患者治疗过程中,有助于改善患者主观不适疼痛感,预防术后鼻腔粘连并发症,提高疾病治愈率,是理想的治疗方案。Objective To analyze the clinical efficacy and safety of low temperature plasma combined with microtamponade in patients with intractable epistaxis.Methods A total of 80 patients with refractory rhinorrhea treated in the otolaryngology Department of Qidong Hospital of Traditional Chinese Medicine from January 2019 to June 2022 were random selected as the research objects,and were divided into two groups according to systematic blind selection method,with 40 patients in each group.The conventional group was treated with nasal endoscopic posterior nostril tamponade+Vaseline gauze tamponade,while the study group was treated with low temperature plasma+microtamponade.The subjective discomfort and nasal adhesion of the two groups were evaluated,and the clinical therapeutic effect was analyzed.Results Group cure rate 97.50%(39/40),significantly higher than the conventional group 80.00%(32/40),the difference was statistically significant(χ^(2)=4.507,P<0.05).In terms of subjective discomfort VAS score,nasal congestion(1.88±2.05)points,nasal distension(2.62±2.31)points,head pain(2.48±1.92)points,tears discharge(2.06±1.44)points,dysphagia(0.73±0.56)points,sleep disorders(2.04±1.83)points in the study group were lower than those in the conventional group(5.20±2.18)points,(5.58±2.25)points,(5.61±2.03)points,(3.77±1.86)points,(5.18±1.39)points,(5.57±2.34)points,and the difference was statistically significant(P<0.05).The incidence of post‐operative nasal adhesions in the study group was 0.00%(0/40),which was lower than that in the conventional group 17.50%(7/40),and the difference was statistically significant(P<0.05).Conclusion The combined application of lowtemperature plasma and micro-tamponade technology in the treatment of patients with intractable nasal bleeding is an ideal treatment plan,which can help to improve patients´subjective discomfort and pain,prevent postoperative complications of nasal adhesions,and improve the cure rate of the disease.
