经脐单切口腹腔镜技术在输卵管妊娠手术中的应用

摘　　要：目的探讨经脐单切口腹腔镜技术在输卵管妊娠手术中的应用效果。方法随机收集2021年4月—2022年8月在涪陵区妇幼保健院接受经脐单切口腹腔镜输卵管手术的输卵管妊娠患者20例与接受常规腹腔镜输卵管手术的输卵管妊娠患者20例,设为研究组、对照组。回顾性分析两组患者的手术情况、术后并发症发生率、术后恢复时间、术后疼痛评分。结果两组手术时间比较,差异无统计学意义(P>0.05);研究组的术中出血量(13.27±3.05)mL、术后首次排气时间(44.91±1.54)h、术后卧床时间(2.79±0.62)d、住院时间(3.11±0.67)d均优于对照组,差异有统计学意义(t=5.713、4.585、4.771、3.970,P<0.05)。研究组的并发症发生率0.00%低于对照组的20.00%,但差异无统计学意义(P>0.05)。研究组术后1、2、3 d的疼痛评分均低于对照组,差异有统计学意义(P<0.05)。结论在输卵管妊娠患者手术治疗时,相比于常规腹腔镜输卵管手术,经脐单切口腹腔镜输卵管手术可在不延长手术时间的前提下,有效减轻患者手术创伤,减少术中出血量,减轻患者术后疼痛感,加快术后康复。Objective To investigate the effect of single incision transumbilical laparoscopy in tubal pregnancy operation.Methods 20 patients with fallopian tube pregnancy who underwent single incision laparoscopic fallopian tube surgery via umbilical cord at Fuling District Maternal and Child Health Hospital from April 2021 to August 2022,and 20 patients with fallopian tube pregnancy who underwent conventional laparoscopic fallopian tube surgery were random selected,and set them as the research group and control group.Retrospective analysis of the surgical situation,incidence of postoperative complications,postoperative recovery time,and postoperative pain score of the two groups of patients.Results There was no statistically significant difference in surgical time between the two groups(P>0.05).The intraoperative bleeding volume(13.27±3.05)mL,first postoperative exhaust time(44.91±1.54)hours,postoperative bed rest time(2.79±0.62)days,and hospitalization time(3.11±0.67)in the study group were all better than those in the control group,the difference was statistically significant(t=5.713,4.585,4.771,3.970,P<0.05).The incidence of complications in the study group was 0.00%,lower than 20.00%in the control group,but the difference was not statistically significant(P>0.05).The pain scores of the study group on the 1st,2nd,and 3rd day after surgery were lower than those of the control group,the difference was statistically significant(P<0.05).Conclusion In the surgical treat‐ment of tubal pregnancy patients,compared with conventional laparoscopic tubal surgery,single transumbilical incision laparoscopic tubal surgery can effectively reduce surgical trauma,reduce intraoperative blood loss,reduce postoperative pain,and accelerate postoperative rehabilitation without prolonging the operation time.
