尿道板纵切卷管尿道成形术应用于尿道下裂治疗的临床效果分析

摘　　要：目的探讨尿道板纵切卷管尿道成形术应用于尿道下裂治疗中的临床效果。方法随机选取2019年6月—2021年6月于山东大学附属儿童医院确诊为尿道下裂的120例患儿为研究对象,以随机分组法分为对照组(60例)和观察组(60例)。对照组开展横裁包皮带蒂岛状皮瓣尿道成形术治疗,观察组开展尿道板纵切卷管尿道成形术治疗。比较两组远期疗效、临床相关指标、并发症发生情况及家长对阴茎外观的满意度。结果观察组的远期治疗有效率(98.33%)高于对照组,差异有统计学意义(χ^(2)=10.439,P<0.05)。观察组在术中出血量(15.79±1.45)mL、手术时间(120.46±1.60)min及住院时间(10.45±1.07)d优于对照组,差异有统计学意义(t=29.440、60.333、14.069,P<0.05)。观察组的术后并发症总发生率(6.67%)低于对照组,差异有统计学意义(χ^(2)=13.333,P<0.05)。观察组患儿家长对阴茎外观的满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论尿道板纵切卷管尿道成形术应用于尿道下裂治疗中有助于提升患儿的远期疗效,改善治疗效果,减少术后并发症发生率,提高患儿家长对阴茎外观的满意度水平。Objective To investigate the clinical effect of longitudinal urethral plate section coiled tube urethroplasty in the treatment of hypospadias.Methods 120 children diagnosed with hypospadias at the Children's Hospital Affiliated to Shandong University from June 2019 to June 2021 were selected randomly as the study subjects.They were randomly divided into a control group(60 cases)and an observation group(60 cases).The control group underwent transverse circumcision with pedicle island skin flap urethroplasty,while the observation group underwent longitudinal urethral plate section coiled tube urethroplasty.Compared the long-term efficacy,clinical related indicators,incidence of complications,and parental satisfaction with penile appearance between the two groups.Results The longterm treatment response rate of the observation group(98.33%)was higher than that of the control group,and the difference was statistically significant(χ^(2)=10.439,P<0.05).The observation group had better intraoperative bleeding volume(15.79±1.45)mL,surgery time(120.46±1.60)min,and hospitalization time(10.45±1.07)d than the control group,and the difference was statistically significant(t=29.440,60.333,14.069,P<0.05).The total incidence of postoperative complications in the observation group(6.67%)was lower than that in the control group,and the difference was statistically significant(χ^(2)=13.333,P<0.05).The satisfaction scores of parents in the observation group for the appearance of the penis were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of longitudinal urethral plate section coiled tube urethroplasty in the treatment of hypospadias can help improve the long-term efficacy of children,improve treatment effectiveness,reduce the incidence of postoperative complications,and increase the satisfaction level of parents with penile appearance.
