单侧双通道内镜下腰椎融合术治疗单节段腰椎退行性疾病的临床疗效

摘　　要：目的探讨针对单节段腰椎退行性疾病患者采用单侧双通道内镜下腰椎融合术的应用价值。方法随机选取2020年6月—2022年4月单县中心医院收治的60例单节段腰椎退行性疾病患者为研究对象,以随机数表法分为两组,每组30例。对照组采取微创通道下腰椎融合术治疗,研究组采取单侧双通道内镜下腰椎融合术,对比手术及术后相关指标、腰椎功能评分、肢体障碍及疼痛评分以及腰椎融合情况。结果研究组手术时间较对照组更长,术中出血量、伤口引流量以及住院时间均较对照组更少,差异有统计学意义(t=5.384、5.559、10.841、4.460,P<0.05)。研究组JOA量表各项评分及总分均高于对照组,差异有统计学意义(t=12.409、8.587、5.926、8.688,P<0.05)。研究组Oswestry评分(22.94±4.31)分、VAS评分(2.53±1.02)分均低于对照组,差异有统计学意义(t=8.435、3.760,P<0.05)。两组腰椎融合情况比较,差异无统计学意义(χ^(2)=0.647,P>0.05)。结论在单节段腰椎退行性疾病患者的治疗中运用单侧双通道内镜下腰椎融合术治疗的效果显著,可减少术中出血量,缩短术后恢复时间,改善腰椎功能,提高生活质量。Objective To explore the application value of unilateral dual-channel endoscopic lumbar fusion for the treatment of patients with single-segment lumbar degenerative diseases.Methods Sixty patients with single-level lumbar degenerative diseases admitted to Shanxian Central Hospital from June 2020 to April 2022 were randomly selected as the research objects,and randomly divided into two groups with 30 cases in each group by number table method.The control group was treated with minimally invasive channel lumbar fusion,while the study group was treated with unilateral double-channel endoscopic lumbar fusion.The surgical and postoperative indexes,lumbar function scores,limb dysfunction and pain scores,and lumbar fusion were compared.Results The operation time of the study group was longer than that of the control group,and the intraoperative blood loss,wound drainage volume and hospital stay were less than that of the control group,and the differences were statistically significant(t=5.384,5.559,10.841,4.460,P<0.05).The scores and total scores of JOA scale in the study group were higher than those in the control group,and the differences were statistically significant(t=12.409,8.587,5.926,8.688,P<0.05).The Oswestry score(22.94±4.31)points and VAS score(2.53±1.02)points of the study group were lower than those of the control group,and the differences were statistically significant(t=8.435,3.760,P<0.05).There was no statistically significant difference in lumbar fusion between the two groups(χ^(2)=0.647,P>0.05).Conclusion In the treatment of patients with single-level lumbar degenerative diseases,unilateral dual-channel endoscopic lumbar fusion has significant effects,which can reduce intraoperative blood loss,shorten postoperative recovery time,improve lumbar function,and improve quality of life.
