运动疗法与自制简易矫形器联用对手部烧伤后瘢痕挛缩患者疗效的影响

摘　　要：目的分析手部烧伤后瘢痕挛缩患者开展自制简易矫形器以及运动疗法干预的作用。方法随机选择2020年3月—2021年10月广西壮族自治区江滨医院物理治疗科收治的手部烧伤后瘢痕挛缩患者60例为研究对象,按照随机摸红蓝球方式分组,其中常规组(30例,运动疗法)摸蓝球,研究组(30例,自制简易矫形器联合运动疗法)摸红球,对比两组各时期手功能测试时间、手部瘢痕得分、日常生活活动能力得分以及关节活动度优良率。结果治疗16周后,两组患者手功能测试时间均短于治疗前,且研究组短于常规组,差异有统计学意义(P<0.05)。治疗16周后,两组患者手部瘢痕得分、日常生活活动能力得分均优于治疗前,且研究组优于常规组,差异有统计学意义(P<0.05)。常规组患者关节活动度优良率70.00%低于研究组的93.33%,差异有统计学意义(χ^(2)=5.455,P<0.05)。结论手部烧伤后瘢痕挛缩患者开展自制简易矫形器以及运动疗法治疗,能够提升手功能以及关节活动度,改善手部瘢痕情况,提高日常生活活动能力。Objective To analyze the effect of homemade simple orthoses devices and exercise therapy interventions on patients with post-burn scar contracture of the hand.Methods Sixty patients with scar contracture after hand burns admitted to the Physical Therapy Department of Jiangbin Hospital in Guangxi Zhuang Autonomous Region from March 2020 to October 2021 were randomly selected as the research subjects.They were randomly divided into two groups:the conventional group(30 cases,exercise therapy)touched the blue ball,and the study group(30 cases,self-made simple orthoses combined with exercise therapy)touched the red ball.The hand function testing time,hand scar scores,and other factors during each period were compared between the two groups Daily activity score and excellent rate of joint mobility.Results After 16 weeks of treatment,the hand function test time of both groups of patients was shorter than before treatment,and the study group was shorter than the conventional group,the difference was statistically significant(P<0.05).After 16 weeks of treatment,the scores of hand scars and activities of daily living in the two groups of patients were better than before treatment,and the study group was better than the conventional group,the difference was statistically significant(P<0.05).The excellent and good rate of joint mobility in the conventional group was 70.00%,lower than 93.33%in the study group,the difference was statistically significant(χ^(2)=5.455,P<0.05).Conclusion Patients with scar contracture after hand burns can receive self-made simple orthoses and exercise therapy to improve hand function and joint mobility,improve hand scar condition,and enhance daily living activities.
