颈浅丛联合肌间沟臂丛阻滞治疗锁骨骨折的价值分析

摘　　要：目的探究锁骨骨折手术采取颈浅丛联合肌间沟臂丛阻滞治疗的临床效果。方法回顾性分析2020年2月—2021年2月福建省漳州市第三医院麻醉科锁骨骨折患者70例临床资料,根据麻醉方法不同分为两组,各35例。常规组采用单纯肌间沟臂丛阻麻醉,观察组采用颈浅丛配合肌间沟臂丛阻麻醉,对比分析两组不同神经阻滞麻醉的效果。结果观察组麻醉总优良率为97.14%高于常规组的77.14%,差异有统计学意义(χ^(2)=4.590,P<0.05)。观察组霍纳综合征、局麻药毒性作用、气胸发生率与常规组比较,差异无统计学意义(P>0.05)。切皮时、骨折复位时,观察组HR与SBP均低于对照组,差异有统计学意义(P<0.05)。结论锁骨骨折手术中运用颈浅丛联合肌间沟臂丛阻滞可获得良好的麻醉效果,且不良反应率低,血流动力学稳定。Objective To investigate the clinical effect of superficial cervical plexus combined with interosseous groove brachial plexus block in clavicle fracture surgery.Methods Retrospective analysis of the clinical data of 70 patients with clavicle fractures in the Anesthesia Department of the Third Hospital of Zhangzhou City,Fujian Province from February 2020 to February 2021.They were divided into two groups based on different anesthesia methods,with 35 patients in each group.The conventional group received simple intramuscular sulcus brachial plexus block anesthesia,while the observation group received superficial cervical plexus combined with intramuscular sulcus brachial plexus block anesthesia.The effects of different nerve block anesthesia in the two groups were compared and analyzed.Results The overall excellent and good rate of anesthesia in the observation group was 97.14%,which was higher than 77.14%in the conventional group,the difference was statistically significant(χ^(2)=4.590,P<0.05).There was no statistically significant difference in the incidence of Horner's syndrome,local anesthetic toxicity,and pneumothorax between the observation group and the conventional group(P>0.05).During skin cutting and fracture reduction,the HR and SBP in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion The use of superficial cervical plexus combined with interosseous sulcus brachial plexus block in clavicle fracture surgery can obtain good anesthetic effect with low adverse reaction rate and stable hemodynamics.
