摘　　要：目的探索2型糖尿病患者(diabetes mellitus type 2,T2DM)与糖尿病肾病(diabetic nephropathy,DN)患者依从性、自我管理与健康相关生命质量(health-related quality of life,HRQoL)的差异,并构建结构方程阐明其相互作用关系。方法选取2019年10月—2020年8月漯河市中心医院确诊的T2DM患者和DN患者各150例,均完成了一系列问卷,同时以结构方程模型为基础展开数据分析。结果DN组患者CN-ADDQoL、PAID-5、SF-36量表分值分别为(0.31±1.08)分、(62.10±18.80)分和(70.30±3.30)分,与T2DM组的(0.62±0.85)分、(36.10±11.80)分和(86.30±3.80)分比较,差异有统计学意义(P<0.05)。结构方程模型结果表明,对糖尿病患者生存质量的最大影响因素为活动和饮食情况(P<0.05)。结论在临床实践中应关注患者的日常活动和饮食情况,制订干预策略,是糖尿病患者生存与生活质量提升的重要因素。Objective To explore the differences in compliance,self-management and health-related quality of life(HRQoL)between patients with type 2 diabetes(diabetes mellitus type 2,T2DM)and patients with diabetic nephropathy(DN),and to build structural equations to clarify their interaction relationship.Methods 150 T2DM patients and 150 DN patients diagnosed at Luohe Central Hospital from October 2019 to August 2020 were selected,and a series of questionnaires were completed.At the same time,data analysis was conducted based on structural equation modeling.Results The scores of CN-ADDQoL,PAID-5,and SF-36 scales in the DN group were(0.31±1.08)points,(62.10±18.80)points,and(70.30±3.30)points,respectively.Compared to the scores of(0.62±0.85)points,(36.10±11.80)points,and(86.30±3.80)points in the T2DM group,the difference was statistically significant(P<0.05).The results of structural equation model showed that the most influential factors on the quality of life of diabetes patients were activity and diet(P<0.05).Conclusion In clinical practice,attention should be paid to patients'daily activities and diet,and intervention strategies should be formulated,which are important factors for the survival and quality of life of diabetic patients.
