摘 要:慢性难愈性创面的修复是外科治疗面临的一个难题,主要在于其治疗周期长且经规范治疗后仍疗效欠佳。研究表明间充质干细胞(MSC)来源的外泌体可以抑制过度炎性反应、促进成纤维细胞和血管内皮细胞的增殖及迁移,调节细胞外基质,在创面愈合的多个阶段均可发挥作用,为临床治疗慢性难愈性创面提供了新的选择。但外泌体在慢性创面修复领域的具体调控机制尚未完全揭示,且如何将其合理利用实现临床精致治疗,是目前所面临的难题。因此,该文主要就MSC来源的外泌体促进慢性难愈性创面修复的机制及其目前的临床应用进行综述。The repair of chronic refractory wounds is a difficult problem for surgical treatment,mainly because of its long treatment cycle and poor efficacy after standardized treatment.Studies have shown that exosomes derived from mesenchymal stem cells(MSCs)can inhibit excessive inflammatory responses,promote the proliferation and migration of fibroblasts and vascular endothelial cells,regulate the extracellular matrix,and play a role in multiple stages of wound healing,which can provide a new option for the clinical treatment of chronic refractory wounds.However,the specific regulatory mechanism of exosomes in the field of chronic wound repair has not yet been fully revealed,and how to rationally use them to achieve clinical exquisite treatment is a difficult problemat present.Therefore,this paper mainly reviews the mechanism and current clinical application of MSC exosomes in promoting the repair of chronic refractory wounds.
