摘　　要：社区主导的糖尿病多学科联合诊疗门诊是建立包括全科医师、临床药师、专职护师、中医师、公共卫生医师和检验师在内的学科团队,通过对患者实施科学、系统和全面的干预,从而达到改善预后和促进康复的目的。本文运用SWOT分析法对开展社区糖尿病多学科联合诊疗门诊进行优势、劣势、机会和风险的系统分析,为提升社区糖尿病患者的健康管理质量提供策略。The community-led diabetes multidisciplinary joint outpatient clinic is a multidisciplinary team,including general practitioners,clinical pharmacists,full-time nurses,traditional Chinese medicine physicians,public health physicians and laboratory technicians,which can improve the prognosis and promote the rehabilitation of patients through scientific,systematic and comprehensive intervention.This paper used SWOT analysis to systematically analyze the strengths,weaknesses,opportunities and threats of developing the community diabetes multidisciplinary joint outpatient clinic,so as to provide strategies for improving the quality of health management for diabetic patients in the community.
