产后逐瘀胶囊辅助治疗对产后出血患者凝血因子及氧化应激的影响

摘　　要：目的 研究产后逐瘀胶囊辅助治疗对产后出血患者凝血因子及氧化应激的影响。方法 选取2019年6月—2021年6月期间河南省南阳市第二人民医院进行分娩并产后出血患者100例,采用随机数字表法分为对照组和观察组,每组各50例。对照组产妇在分娩后的治疗方案为米索前列醇联合欣母沛,观察组在对照组的治疗方案上加用产后逐瘀胶囊。连续治疗两周后,观察比较两组患者临床疗效,检测两组患者治疗前后凝血因子[纤维蛋白原(Fibrinogen, FIB)、D-二聚体水平(D-Dimer Calibrator, D-D)]、氧化应激指标[超氧化物歧化酶(Superoxide dismutase, SOD)、谷胱甘肽过氧化物酶(Glutathione peroxidase, GSH-Px)和脂质过氧化物(Lipid peroxidation, LPO)]及出血情况。结果 治疗后观察组临床总有效率96.00%(48/50)明显高于对照组80.00%(40/50),差异有统计学意义(P<0.05)。治疗后两组患者凝血因子FIB、D-D水平较治疗前降低,差异有统计学意义(P<0.05);且观察组凝血因子FIB、D-D水平低于对照组,差异有统计学意义(P<0.05)。治疗后两组血清GSH-P、SOD水平较治疗前升高,LPO水平较治疗前降低,差异有统计学意义(P<0.05);且观察组血清GSH-P、SOD水平高于对照组,LPO水平低于对照组,差异有统计学意义(P<0.05)。观察组出血量、出血时间及后期出血例数明显低于对照组,差异有统计学意义(P<0.05)。结论 产后逐瘀胶囊止血效果良好,有效调节凝血因子水平及机体氧化应激反应。Objective To observe the effects of Chanhou Zhuyu Capsules on coagulation factors and oxidative stress in the patients with postpartum hemorrhage.Methods One hundred patients with postpartum hemorrhage treated in Nanyang Second General Hospital from June 2019 to June 2021 were selected and assigned into observation and control groups(n=50).The control group was treated with misoprostol combined with hemabate after delivery,while the observa-tion group additionally with Chanhou Zhuyu Capsules.The clinical effects of the two groups were compared after two weeks of treatment.The coagulation factors[fibrinogen(FIB)and D-dimer calibrator(D-D)]and oxidative stress indicators[su-peroxide dismutase(SOD),glutathione peroxidase(GSH-Px),and lipid peroxide(LPO)]were measured before and after treatment,and the bleeding situations in the two groups were recorded.Results The total effective rate of the observation group was 96.00%(48/50),which was higher than that(80.00%,40/50)of the control group(P<0.05).After treat-ment,the levels of FIB and D-D in both groups declined compared with those before treatment,and the levels in the ob-servation group were lower than those in the control group(P<0.05).The treatment in both groups elevated the GSH-Px and SOD levels and lowered the LPO level in the serum(P<0.05).Moreover,the observation group had higher GSH-Px and SOD levels and lower LPO level than the control group after treatment(P<0.05).The blood loss,bleeding time,and the number of late bleeding patients in the observation group were lower than those in the control group(P<0.05).Con-clusion Chanhou Zhuyu Capsules demonstrates good hemostatic effect and effectively regulates the levels of coagulation factors and the oxidative stress response.
