摘 要:目的探讨奇静脉食管隐窝(AER)多层螺旋CT(multi-slice spiral computed topography,MSCT)不同测量方法优劣。方法收集中老年人群MSCT胸部体检资料,依据有无右侧肺气肿分成两组,右侧肺气肿分轻、中、重三度。AER宽度测量:方法1:胸椎前缘平行于基线的切线与右主支气管后壁间距为W1;方法2:切线与AER前壁间距为W2。深度测量:方法1:AER顶点到胸骨后缘中点和胸椎前缘凸点连线间距离为D1;方法2:与胸椎正中矢状线间距为D2。AER顶点过连线为深型,否则为浅型。AER径线大小比较行t检验,与肺气肿程度相关性及其分级关系分别采用Spearman秩相关分析评价及单因素方差分析。结果肺气肿组深型AER(41例)的W2平均值为(25.78±9.77)mm,显著大于W1平均值:(23.15±8.11)mm(t=4.39,P<0.05);W2与肺气肿程度呈正相关(r=0.316,P<0.05),且与其分级显著相关(F=5.85,P<0.05)。肺气肿组深型AER(41例)的D2平均值为(9.84±4.01)mm,显著大于D1平均值:(7.37±3.36)mm(t=4.86,P<0.05);D2与肺气肿程度呈正相关(r=0.415,P<0.05),且与其分级显著相关(F=6.14,P<0.05)。肺气肿组浅型AER(13例)及无肺气肿组无论深浅型AER(分别为24例、146例)的W2与W1和D2与D1差异均无统计学意义(P>0.05)。结论AER方法2测量结果反映气道内阻力情况较敏感,且不受胸骨偏移影响,优于方法1。Objective To compare the advantages and disadvantages of different methods of Multi-slice spiral computed tomography(MSCT)measurement of the azygoesophageal recess(AER).Methods Data of middle-aged elderly people undergoing chest physical examination were randomly collected and divided into two groups:emphysema of the right lung and no emphysema of the right lung.Right emphysema was classified as mild,moderate,and severe.The measurement of the width of AER with method1:The distance between the tangent line of the anterior edge of the thoracic vertebra parallel to the baseline and the right bronchus was W1;method2:The distance between the tangent line of the anterior edge of the thoracic vertebra and the front wall of AER was W2.Depth with method1:The distance of the vertex of AER to the line between the midpoint of the posterior margin of the sternum and the convex point of the anterior margin of the thoracic vertebra was D1;method2:The distance between the vertex of AER to the midsagittal diameter of the thoracic vertebra was D2.The type of AER was deep if its vertex was over the line,otherwise shallow.The measurement results of different methods between groups of the size of AER were tested by t-test.Spearman rank correlation analysis and one-way analysis of variance were used to evaluate the correlation between emphysema degree and grade.Results The mean value of W2 in the emphysema group(41 cases)was(25.78±9.77)mm,which was significantly higher than the mean value of W1(23.15±8.11)mm(t=4.39,P<0.05).W2 was positively correlated with the degree of emphysema(r=0.316,P<0.05),and was significantly correlated with its grade(F=5.85,P<0.05).In the emphysema group(41 cases),the mean value of D2 was(9.84±4.01)mm,which was significantly higher than the mean value of D1:(7.37±3.36)mm(t=4.86,P<0.05).D2 was positively correlated with the degree of emphysema(r=0.415,P<0.05),and was significantly correlated with its grade(F=6.14,P<0.05).There was no significant difference between W2 and W1,D2 and D1 in the emphysema group
