社区慢性病精细化管理对老年糖尿病患者的应用效果分析

摘　　要：目的:探讨社区慢性病精细化管理对老年糖尿病患者的应用效果。方法:选取2021年4—6月北京市东城区东花市社区卫生服务中心收治并管理的196例老年糖尿病患者作为研究对象,采用随机数字表法分为对照组与观察组,各98例。对照组实施常规社区慢性病管理,观察组实施社区慢性病精细化管理。比较两组干预前后血压、血糖、血脂指标。结果:干预后,两组收缩压、舒张压均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。干预后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。干预后,两组总胆固醇、甘油三酯水平均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:社区慢性病精细化管理对老年糖尿病患者的应用效果显著,可更好地改善患者血压、血糖、血脂水平,建议推广。Objective:To explore the application effect of community refined management of chronic diseases in elderly patients with diabetes.Methods:A total of 196 elderly patients with diabetes who were admitted to and managed by Beijing Dongcheng District Donghuashi Community Health Service Center from April 2021 to June 2021 were selected as the study subjects.They were divided into the control group and the observation group by using the random number table,with 98 cases in each group.The control group implemented routine community management of chronic disease and the observation group implemented community refined management of chronic disease.The blood pressure,blood glucose and blood lipid indexes before and after the intervention were compared between the two groups.Results:After the intervention,the systolic blood pressure and diastolic blood pressure in the two groups were lower than those before the intervention,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).After the intervention,the fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels in the two groups were lower than those before the intervention,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).After the intervention,the total cholesterol and triglyceride levels in the two groups were lower than those before the intervention,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion:The refined management of chronic diseases in the community for elderly patients with diabetes has significant application effect,which can better improve the levels of blood pressure,blood glucose and blood lipid of patients,and it is recommended for promotion.
