摘　　要：目的:探讨后外侧联合后内侧切口双窗口入路治疗胫骨平台后柱骨折的临床效果。方法:选取2017年6月—2021年6月如东县人民医院收治的60例胫骨平台后柱骨折患者作为研究对象,根据随机抽签法分为对照组和观察组,各30例。对照组行后内侧切口入路切开复位内固定术治疗,观察组行后外侧联合后内侧切口双窗口入路切开复位内固定术治疗。比较两组术后恢复时间、治疗效果、并发症发生情况。结果:观察组术后消肿时间、住院时间、骨痂出现时间、负重时间均短于对照组,差异有统计学意义(P<0.001)。观察组治疗总优良率高于对照组,差异有统计学意义(P=0.037)。观察组并发症发生率低于对照组,差异有统计学意义(P=0.019)。结论:后外侧联合后内侧切口双窗口入路治疗胫骨平台后柱骨折的效果显著,可保证手术安全性,利于术后尽早恢复,改善膝关节功能,值得推广。Objective:To investigate the clinical effect of posterolateral combined posteromedial incision double-window approach in the treatment of posterior column fractures of the tibial plateau.Methods:A total of 60 patients with posterior column fractures of the tibial plateau who were admitted to Rudong County People's Hospital from June 2017 to June 2121 were selected as the study subjects.They were divided into control group and observation group by random lottery method,with 30 cases in each group.The control group underwent open reduction and internal fixation through posteromedial incision approach,and the observation group underwent open reduction and internal fixation through posterolateral combined posteromedial incision double-window approach.The postoperative recovery time,the treatment effect and the occurrence of complications were compared between two groups.Results:The postoperative swelling subsiding time,hospitalization time,bony callus appearance time,and weight-bearing time in the observation group were shorter than those in the control group,with statistically significant difference(P<0.001).The excellent and good rate of treatment in the observation group was higher than that in the control group,with statistically significant difference(P=0.037).The incidence of complications in the observation group was lower than that in the control group,with statistically significant difference(P=0.019).Conclusion:The posterolateral combined posteromedial incision double-window approach has significant effects in the treatment of posterior column fractures of the tibial plateau,it can ensure the operation safety,speed up postoperative recovery,and improve knee joint function.It deserves the promotion.
