摘 要:呼吸机输出潮气量设置值和显示值的准确性是呼吸机基本性能的关键组成部分,直接影响到重症监护室机械通气的安全性和有效性。文章参考重症护理呼吸机质量检测新的国家标准GB 9706.212-2020推荐的测试方法,对两台临床常见的重症呼吸机S1和S2潮气量控制公差DoVsa和监测精度DoVma进行了测试。结果发现,样品S1在输出潮气量设置Vs=50mL时,使用具有特定线性阻力R和顺应性C组合值的模拟肺测试,导致DoVsa超出制造商声称的潮气量控制精度;两台被测呼吸机在Vs=30mL时,使用R值最大且C值最小的模拟肺测试时,对应的DoVsa最小。进一步推测,呼吸机流量传感器不同流量范围内非线性误差不一致,可能是影响不同R和C组合时传输潮气量控制精度的主要因素,新标准的颁布实施有利于呼吸机关键部件及其控制算法的技术创新。The accuracy of set and displayed tidal volumes delivered is a key component of the essential performance of a ventilator,which directly affects the safety and effectiveness of mechanical ventilation in ICU.In this paper,we tested the control tolerance of tidal volumes delivered(DoVsa)and the accuracy of the delivered volume monitoring equipment(DoVma)for two critical care ventilators,S1 and S2,commonly used in clinical settings,by using the test method recommended by GB 9706.212–2020,a new national standard for the quality testing of critical care ventilators.It was found that when the delivered tidal volume(Vs)was set at 50mL and the test lung was configured with the specific linear resistance(R)and compliance(C)components,the DoVsa of the sample S1 exceeded the control accuracy of the delivered tidal volume as claimed by the manufacturer;when Vs was set at 30mL and the test lung with the maximum R and the minimum C was used,the corresponding DoVsa was the smallest value.We further speculate that the inconsistent nonlinear errors of the ventilator flow sensor within different flow ranges may be the main factor affecting the control accuracy of the delivered tidal volume in different combination of R and C.The promulgation and implementation of the new standard is instrumental in the technical innovation of the key components and their control algorithms for ventilators.
