多层螺旋CT冠状动脉造影与数字减影血管造影诊断冠脉支架血管再狭窄的效果观察

摘　　要：目的:探析多层螺旋CT冠状动脉造影与数字减影血管造影(DSA)诊断冠脉支架血管再狭窄的效果。方法:将2020年1月~2022年1月本院收治的冠状动脉支架植入术患者80例作为研究对象,对其实施DSA诊断与多层螺旋CT冠状动脉造影,统计两组特异度、准确率与敏感度等相关指标。结果:80例冠脉支架血管再狭窄患者可清晰显示支架240枚,其中左前降支、左回旋支与对角支分别是120枚、80枚以及40枚。通过多层螺旋CT冠状动脉造影可以了解冠脉支架血管再狭窄患者的冠状动脉支架情况,尤其是冠状动脉支架内血管情况,如钙化斑块、非钙化斑块及其狭窄程度。多层螺旋CT冠状动脉造影检查诊断发现支架内血管再狭窄239枚,经DSA诊断证实为235个,多层螺旋CT诊断冠状动脉支架术后血管再狭窄的准确率80.00%、敏感度82.63%、特异度87.32%,Kappa检验值为0.819,多层螺旋CT组与DSA组在多指标(敏感度、准确度与特异度)的对比上基本保持一致。结论:多层螺旋CT冠状动脉造影与DSA诊断冠脉支架血管再狭窄,其特异度、准确度与敏感度差别不大,冠脉支架血管再狭窄对支架血管位置与管腔情况可准确观察,为冠脉支架血管再狭窄治疗提供必要支持。Objective:To explore the effect of multi-slice spiral CT coronary angiography and DSA in the diagnosis of coronary stent restenosis.Methods:From January 2020 to January 2022,80 patients with coronary artery stenting were treated in our hospital as the subjects of this study.DSA diagnosis and multi-slice spiral CT coronary angiography were performed on them,and the specificity,accuracy and sensitivity of the two groups were statistically analyzed.Results:In 80 patients with coronary stent restenosis,240 stents can be clearly displayed,including 120 left anterior descending branch,80 left circumflex branch and 40 diagonal branches.Multi-slice spiral CT coronary angiography can be used to understand the condition of coronary stents in patients with coronary stent restenosis,especially the condition of vessels in coronary stents,such as calcified plaque,non-calcified plaque and the degree of stenosis.Multi-slice spiral CT coronary angiography diagnosis found 239 in-stent restenosis,and confirmed by DSA diagnosis was 235.The accuracy rate,sensitivity and specificity of multi-slice spiral CT diagnosis of coronary restenosis after coronary stenting were 80.00%,82.63%and 87.32%,and Kappa test value was 0.819.Multi-slice spiral CT group and DSA group were basically consistent in the comparison of multiple indicators(sensitivity,accuracy and specificity).Conclusion:Multi-slice spiral CT coronary angiography and DSA have little difference in specificity,accuracy and sensitivity in diagnosing coronary stent restenosis.Coronary stent restenosis can accurately observe the position and lumen of the stent,providing necessary support for the treatment of coronary stent restenosis.
