摘 要:目的:探讨研究全自动血细胞分析仪联合血涂片细胞形态学在血常规检验中的应用效果。方法:选取2019年1月~2020年1月本院就诊患者血液样本480份,所有样本均接受血常规检验;所有血液样本全部都进行血涂片细胞形态学及全自动血细胞分析仪检验。观察并比较两种不同检验方式的阳性检出比率水平差异;两种不同检验方式在不同检验项目下的符合率水平。结果:全自动分析仪的阳性检出比率水平比血涂片检查显著更高(P<0.05)。在全部发生报警的血液标本当中,镜检结果阳性检出率为320份(66.7%),其中,56例为原始细胞、70例为异型淋巴细胞、66例为核左移、56例为有核红细胞、70例为血小板减少、68例为血小板升高;存在两种以上情况的异常标本有84例(26.3%)。针对没有出现报警的160份标本使用显微镜进行观察,可见白细胞计数,单核粒细胞:62.5%、幼稚粒细胞:50.0%、形态异常RBC:37.5%、原始细胞:33.3%、嗜酸性粒细胞:28.6%、杆状粒细胞:20.0%;总符合率为36.7%,相比血细胞分析仪的检验结果无显著性的差异(P>0.05)。结论:在临床血常规检验当中广泛性的使用细胞形态学检验及全自动血细胞分析仪检验,互补并联合二者在临床当中的应用优势,可显著提高血常规检验的准确率,从而为临床对相关疾病的诊断及治疗提供更加准确且可靠的参考资料。Objective:To explore the application effect of automatic blood cell analyzer combined with blood smear cell morphology in blood routine test.Methods:480 blood samples from January 2019 to January 2020 in our hospital were selected,all samples were subject to blood routine test;All blood samples were tested by blood smear cell morphology and automatic blood cell analyzer.Observe and compare the difference of positive detection rate of two different test methods;The coincidence rate of two different test methods under different test items.Results:The positive detection rate level of the fully automated analyzer was significantly higher than that of the blood smear test(P<0.05).Among all the blood samples with alarm,the positive detection rate of microscopic examination was 320(66.7%),including 56 cases of archaeocyte,70 cases of heterotypic lymphocytes,66 cases of left shift of nuclei,56 cases of nucleated red blood cells,70 cases of thrombocytopenia,and 68 cases of elevated platelets;There were 84 cases(26.3%)of abnormal specimens with two or more conditions.160 samples without alarm were observed under microscope,and white blood cell counts were found:monocytes:62.5%,immature granulocytes:50.0%,morphologically abnormal RBC:37.5%,archaeocyte:33.3%,eosinophils:28.6%,rod-shaped granulocytes:20.0%;The total compliance rate was 36.7%,and there was no significant difference compared to the test results of the blood cell analyzer(P>0.05).Conclusion:In clinical blood routine test,extensive use of cell morphology test and automatic blood cell analyzer test,complement and combine the advantages of the two in clinical application,can significantly improve the accuracy of blood routine test,so as to improve the clinical diagnosis and treatment of related diseases more accurate and reliable reference materials.
