子痫前期血清炎性因子检测的诊断价值

摘　　要：目的探讨降钙素原(PCT)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、基质金属蛋白3(MMP3)联合检测对子痫前期的诊断价值。方法选择2020年9月至2021年9月在该院产前检查具有子痫前期高危因素的384例孕妇进行前瞻性队列研究,早、中、晚孕期检测各血清炎性因子,比较子痫前期组(n=69)与非子痫前期组(n=315)的表达水平差异,使用受试者工作特征(ROC)曲线寻找诊断效能高的炎性因子。结果69例(18.00%)子痫前期孕妇中,轻度子痫前期55例;重度子痫前期14例,其中HELLP综合征3例。比较早、中、晚孕期子痫前期组与非子痫前期组各血清炎性因子的表达水平:PCT、IL-6、CRP均升高;MMP3降低。PCT的表达升高差异有统计学意义(P<0.05);IL-6、CRP仅在晚孕期表达具有显著性差异。使用ROC曲线评估晚孕期PCT、IL-6、CRP联合检测对子痫前期诊断效能:曲线下面积(AUC)为0.739,灵敏度和特异度分别为75.40%和62.90%,阳性预测值和阴性预测值分别为30.77%和92.10%。结论PCT对子痫前期疾病的诊断具有良好的价值。Objective To explore the diagnostic value of combined detection of procalcitonin(PCT),interleukin-6(IL-6),C-reactive protein(CRP)and matrix metalloprotein 3(MMP3)in preeclampsia.Methods A prospective cohort study was conducted about 384(315+69)pregnant women with high-risk factors of preeclampsia who were selected from prenatal examination in Beijing chaoyang maternal and child health hospital from September 2020 to September 2021.Serum inflammatory factors were detected in early,middle and late pregnancy.The expression levels of preeclampsia group(n=69)and non preeclampsia group(n=315)were compared,the receiver operating characteristic curve(ROC curve)was used to find the inflammatory factors with high diagnostic efficiency.Results Among 69(18.00%)pregnant women with preeclampsia,55 cases were mild preeclampsia;14 cases were severe preeclampsia,including 3 cases of HELLP syndrome.The expression levels of serum inflammatory factors in preeclampsia group and non preeclampsia group in early,middle and late pregnancy were compared:PCT,IL-6 and CRP increased;MMP3 decreased.There was a statistically significant difference in the expression of PCT(P<0.05);The expression of IL-6 and CRP only showed significant differences in late pregnancy.The use of ROC curve to evaluate the diagnostic efficacy of combined detection of PCT,IL-6,and CRP in late pregnancy for preeclampsia:the area under the curve(AUC)was 0.739,the sensitivity and specificity were 75.40%and 62.90%,respectively,and the positive and negative predictive values were 30.77%and 92.10%,respectively.Conclusion PCT has good value in the diagnosis of preeclampsia.
