酶联免疫吸附法与放射免疫法检测17羟孕酮的结果比较

摘　　要：目的分析酶联免疫吸附法与放射免疫法检测17羟孕酮的结果,探讨其灵敏度和特异度的差异,为临床医生正确使用、解读报告单提供有利的诊断依据。方法用酶联免疫吸附法与放射免疫法分别检测40例先天性肾上腺皮质综合征患者和50例健康对照者血清中的17羟孕酮,观察其灵敏度和特异度;用统计学方法分析两种方法的差异性。结果酶联免疫吸附法在先天性肾上腺皮质患者中测定17羟孕酮的灵敏度为90%,特异度为100%,放射免疫分析法在先天性肾上腺皮质患者中测定17羟孕酮的灵敏度为95%,特异度为100%。结论酶联免疫吸附法和放射免疫法检测17羟孕酮,二种方法结果之间的灵敏度和特异度差异均无统计学意义。二者结果之间的一致性非常高,对于一些临床上开展不了放免法的医院,开展酶联免疫吸附法也能够满足临床质量要求,同时酶联免疫吸附法较放射法操作简便,对人体无害,更具有可操作性。另外,临床医生在看不同医院结果的时候,还需了解方法学之间的差异性,综合临床诊断与其他检测项目并进行综合分析,才能对患者做出更好的诊断与治疗。Objective To analyze the results of enzyme-linked immunosorbent assay(ELISA)and radioimmunoassay(RIA)in the detection of 17 Hydroxyprogesterone,explore the difference in sensitivity and specificity,and provide favorable diagnostic basis for clinicians to correctly use and interpret the report.Methods Detected the 17 hydroxyprogesterone with the two methods in 40 cases of Congenital adrenal cortical syndrome patients and 50 cases of healthy people,Compare the sensitivity and specificity and analyse the differences with Statistical methods.Results The sensitivity and the specificity on detecting 17 hydroxyprogesterone in Congenital adrenal cortical syndrome patients with the ELISA were separately 90%and 100%,however,With the RIA,The sensitivity and the specificity separately were 95%and 100%.Conclusion There is no great differences between the ELISA and RIA,The results between the two methods have the highly consistency.Which shows that some hospitals only did ELISA may meet cilical needs.At the same time,ELISA,as a detected methods,operate simply and has no harmless to the body.In addition,while the clinical doctors read the report,who should know the differences among the methodology,only this can we give patients a better diagnosis and treatment.
