经阴道与经腹部超声鉴别诊断子宫内膜良恶性病变的价值分析

摘　　要：目的探讨经阴道与经腹部超声鉴别诊断子宫内膜良恶性病变的价值。方法90例子宫内膜病变患者均进行经阴道超声与经腹部超声检查,分析两种方法诊断子宫内膜病变与病理结果的一致性,比较两种方法的诊断效能。结果Kappa检验显示,经腹部超声检查结果与手术病理检查结果有中等一致性(Kappa值=0.561,P<0.05),经阴道超声检查结果与手术病理检查结果有高度一致性(Kappa值=0.873,P<0.05)。经阴道超声鉴别诊断子宫内膜良恶性病变的灵敏度、特异性及准确率均高于经腹部超声(P<0.05)。结论经阴道超声可准确鉴别子宫内膜病变患者的病理特征,与病理结果的一致性较好,诊断效能高。Objective To explore the value of transvaginal ultrasound and transabdominal ultrasound in differential diagnosis of benign and malignant endometrial lesions.Methods 90 patients with endometrial lesions were examined by transvaginal ultrasound and transabdominal ultrasound.The consistency between the two methods in the diagnosis of endometrial lesions and pathological results was analyzed,and the diagnostic efficacy was compared between the two methods.Results Kappa test showed that the results of transabdominal ultrasound examination were moderately consistent with the results of surgical pathology(Kappa value=0.561,P<0.05),and the results of transvaginal ultrasound were highly consistent with the results of surgical pathology(Kappa value=0.873,P<0.05).The sensitivity,specificity and accuracy of transvaginal ultrasound in differential diagnosis of benign and malignant endometrial lesions were higher than those of transabdominal ultrasound(P<0.05).Conclusions Transvaginal ultrasound can accurately identify the pathological features of patients with endometrial lesions,and has good consistency with pathological results and high diagnostic efficiency.
