摘 要:目的建立早产儿支气管肺发育不良(BPD)风险预测模型,分析早期补充维生素D的干预效果。方法选取2015年7月—2020年3月温州市中心医院收治的早产儿160例纳入研究,将出生后28 d仍需用氧的78例作为BPD组,无需用氧的82例作为非BPD组。采用多因素logistic回归分析法分析早产儿BPD的影响因素。结果logistic回归分析结果显示:剖宫产、胎龄<30周、出生体质量<1500 g、气管插管、新生儿呼吸窘迫综合征、动脉导管未闭、肺动脉高压、新生儿窒息及肺出血是早产儿BPD的危险因素(均OR>1,均P<0.05)。早产儿BPD风险预测模型的ROC曲线下面积(AUC)为0.781。结论剖宫产、胎龄<30周、出生体质量<1500 g、气管插管、新生儿呼吸窘迫综合征、动脉导管未闭、肺动脉高压、新生儿窒息及肺出血是早产儿BPD的危险因素,提早补充维生素D可有效改善BPD早产儿的预后。Objective To establish a risk prediction model for bronchopulmonary dysplasia(BPD)in premature infants,and analyze the intervention effect of early vitamin D supplementation.Methods A total of 160 cases of premature infants admitted to Wenzhou Central Hospital from July 2015 to March 2020 were selected and included in the study,then 78 cases who still needed oxygen at 28 days after birth were divided into BPD group,82 cases of patients who did not need oxygen were regarded as non-BPD group.Multivariate logistic regression analysis was used to analyze the influencing factors of the influencing factors of BPD in premature infants.Results The results of logistic regression analysis showed that cesarean section,gestational age<30 weeks,birth weight<1500 g,tracheal intubation,neonatal respiratory distress syndrome,patent ductus arteriosus,pulmonary hypertension,neonatal asphyxia,and pulmonary hemorrhage were the risk factors of BPD in premature infants(OR>1,P<0.05).The area under ROC curve was 0.781.Conclusion Cesarean section,gestational age<30 weeks,birth weight<1500 g,tracheal intubation,neonatal respiratory distress syndrome,patent ductus arteriosus,pulmonary hypertension,neonatal asphyxia,and pulmonary hemorrhage are the risk factors of BPD in preterm infants.Early vitamin D supplementation can effectively improve prognosis of premature infants with BPD.
