肌间隙入路手术与传统入路手术治疗巨大甲状腺肿的疗效比较

摘　　要：目的比较胸锁乳突肌内缘肌间隙入路与传统入路行巨大甲状腺肿切除手术的效果。方法回顾性分析2018年1月至2022年7月郑州大学附属洛阳中心医院收治的21例经胸锁乳突肌与带状肌间隙入路完成的巨大甲状腺肿手术患者(观察组)与20例传统的离断颈前肌群的帘状开窗入路患者(对照组)的临床资料。比较两组手术情况、并发症发生情况以及术后生活质量。结果与对照组比较,观察组手术时间更短,创伤更小,术后引流少,恢复快,颈前区不适感更轻,治疗效果满意。结论胸锁乳突肌内缘肌间隙入路行巨大甲状腺肿切除安全可行。Objective To compare the effect of sternocleidomastoid intermuscular space approach and traditional approach for giant goiter resection.Methods The clinical data of 21 patients who underwent giant goiter surgery through the sternocleidomastoid and ribbon muscle space approach(observation group)in Luoyang Central Hospital Affliated to Zhengzhou and 20 patients who underwent the traditional curtain-like fenestration approach to amputate the anterior cervical muscles(control group)were analyzed.The surgical conditions,complications,and postoperative quality of life between the two groups were compared.Results Compared with the control group,the observation group had shorter surgical time,smaller trauma,less postoperative drainage,faster recovery,less discomfort in the anterior cervical area,and satisfactory treatment results.Conclusions It is safe and feasible to resect giant goiter through the approach of the internal margin muscle space of sternocleidomastoid muscle.
