益气活血通脉汤联合早期个体化康复训练对股骨粗隆间骨折患者骨折愈合及功能恢复的影响

摘　　要：目的探究益气活血通脉汤联合早期个体化康复训练对股骨粗隆间骨折患者骨折愈合及功能恢复的影响。方法选取2019年9月-2020年9月该院收治的106例股骨粗隆间骨折患者为研究对象,使用随机数字表法分为对照组(53例)和观察组(53例)。对照组患者予以早期个体化康复训练,观察组患者在对照组基础上予以益气活血通脉汤治疗。探究两组患者治疗前、治疗3个月后骨折愈合(股骨头局部骨密度、平均骨密度)、中医证候积分差异,比较两组患者治疗前、治疗3个月后、治疗6个月后功能恢复[髋关节评分(Harris评分)]差异,分析两组患者治疗期间并发症发生差异。结果治疗3个月后,两组患者股骨头局部骨密度、平均骨密度均较治疗前升高,且观察组均高于同期对照组(P均<0.05);两组患者下肢浮肿、气短息弱、刺痛、神疲乏力得分均较治疗前降低,且观察组均低于同期对照组(P均<0.05);治疗3个月后、治疗6个月后,两组患者Harris评分均较治疗前上升,且治疗3个月后、治疗6个月后时观察组患者Harris评分均高于对照组(P均<0.05)。治疗期间,观察组患者深静脉血栓发生率低于对照组(P<0.05),两组患者胃肠道反应、白细胞上升、伤口感染发生率差异均无统计学意义(均P>0.05)。结论气活血通脉汤联合早期个体化康复训练可促进股骨粗隆间骨折患者骨折愈合及功能恢复,减轻患者中医症状,预防深静脉血栓发生。Objective To explore the effect of Yiqi Huoxue Tongmai Decoction(益气活血通脉汤)combined with early individualized rehabilitation training on fracture healing and functional recovery in patients with femoral intertrochanteric fractures.Methods A total of 106 patients with femoral intertrochanteric fractures who were admitted to the hospital between September 2019 and September 2020 were selected as the research subjects,and they were divided into the control group(53 cases)and the observation group(53 cases)by using the random number table method.Patients in the control group received early individualized rehabilitation training and those in the observation group were treated with Yiqi Huoxue Tongmai Decoction on the basis of the control group.The differences in fracture healing(femoral head local bone mineral density,average bone mineral density)and TCM syndromes scores were explored in the two groups before treatment and after 3 months of treatment,and the functional recovery[hip joint score(Harris score)]of the two groups was compared before treatment,after 3 and 6 months of treatment.The differences in occurrence of complications were compared between the two groups during the treatment.Results After 3 months of treatment,the local bone mineral density and average bone mineral density of femoral head of the two groups were increased compared to those before treatment,and the two indexes in the observation group were higher than those in the control group during the same period(all P<0.05).The scores of lower limb edema,shortness of breath,stabbing pain and fatigue were all decreased in the two groups compared with those before treatment,and the scores of the observation group were lower than those of the control group during the same period(all P<0.05).After 3 and 6 months of treatment,the Harris scores of the two groups were higher than those before treatment,and the Harris scores in the observation group were higher than those in the control group after 3 and 6 months of treatment(all P<0.05).During
